【摘要】：目的探討結(jié)直腸癌患者血漿循環(huán)游離DNA(cfDNA)與腫瘤組織DNA的RAS基因突變檢測的一致性,以及與臨床特征的關(guān)系。方法收集71例結(jié)直腸癌患者的血漿及腫瘤組織標(biāo)本。采用多重?zé)晒釶CR法檢測血漿標(biāo)本cfDNA的RAS的基因突變狀態(tài),并與腫瘤組織標(biāo)本的相應(yīng)基因突變狀態(tài)對(duì)比分析。分析血漿RAS基因突變狀態(tài)與患者臨床特征的關(guān)系。結(jié)果血漿cfDNA的RAS突變率為32.39%,組織DNA的RAS突變率為50.70%,兩種標(biāo)本的RAS突變檢測的一致率為76.06%,血漿cfDNA與組織DNA的RAS突變檢測結(jié)果一致(Kappa=0.523,P=0.000)。血漿cfDNA檢測的敏感性為58.33%、特異性為94.29%、陽性預(yù)測值為91.30%、陰性預(yù)測值為68.75%。血漿cfDNA的RAS突變狀態(tài)與結(jié)直腸癌患者年齡、性別、分期(Ⅳa與Ⅳb)、原發(fā)腫瘤部位、肺轉(zhuǎn)移、CEA水平、CA199水平等無明顯相關(guān)性,而血漿cfDNA的RAS突變與結(jié)直腸癌患者肝臟轉(zhuǎn)移有相關(guān)性(P=0.045),肝轉(zhuǎn)移患者較未發(fā)生肝轉(zhuǎn)移的患者突變率升高。結(jié)論血漿cfDNA可以作為無法獲得組織標(biāo)本的結(jié)直腸癌患者的RAS基因檢測的替代標(biāo)本,具有臨床應(yīng)用價(jià)值,且結(jié)直腸癌患者肝臟轉(zhuǎn)移cfDNA RAS突變率升高。
[Abstract]:Objective to investigate the relationship between plasma circulating free DNA-cfDNA and RAS gene mutation in tumor tissues and its relationship with clinical features in colorectal cancer patients.Methods Plasma and tumor tissue specimens were collected from 71 patients with colorectal cancer.Multiplex fluorescence PCR method was used to detect the mutation status of RAS gene in plasma cfDNA, and the mutation status of RAS was compared with that of tumor tissue.To analyze the relationship between the mutation status of plasma RAS gene and the clinical characteristics of patients.Results the RAS mutation rate of plasma cfDNA was 32.39 and the RAS mutation rate of tissue DNA was 50.70. The consistent detection rate of RAS mutation in two kinds of samples was 76.06. The results of RAS mutation detection of plasma cfDNA and tissue DNA were in agreement with that of Kappa (0.523) P 0.000.The sensitivity, specificity, positive predictive value and negative predictive value of plasma cfDNA were 58.33, 94.29, 91.30 and 68.75, respectively.There was no significant correlation between plasma cfDNA mutation status and age, sex, stage (鈪，
