乳腺癌患者外周血循環(huán)腫瘤細胞陰性富集法檢測及其臨床意義

【摘要】:目的循環(huán)腫瘤細胞(circulating tumor cells,CTC)的檢測作為一種新興的檢測方法,在臨床實踐中擁有廣闊的應用前景。本研究通過陰性富集法檢測乳腺癌患者外周血CTC,探討CTC水平與其分子分型及臨床特征的相關性,研究其臨床意義。方法選取連云港市第一人民醫(yī)院2015-08-18-2016-02-20接受CTC檢測的乳腺癌患者152例為研究對象,收集臨床資料,選擇同期該院29例乳腺良性疾病患者作為對照組。采用免疫磁珠陰性富集及免疫熒光原位雜交技術(fluorescence in situ hybridization,FISH)檢測CD45~-/CEP~+/DAPI~+的CTC,并結(jié)合乳腺癌分子分型和臨床特征進行統(tǒng)計學分析。結(jié)果 152例乳腺癌患者與29例乳腺良性疾病患者進行ROC曲線分析,比較不同切點的敏感度及特異度,經(jīng)計算Youden指數(shù)最高為0.390,對應切點為≥2.5,故取CTC個數(shù)≥3為陽性指標。152例乳腺癌患者中CTC陽性率Ⅰ~Ⅱ期為41.5%,Ⅲ期為60.0%,Ⅳ期為73.9%,差異有統(tǒng)計學意義,χ~2=9.360,P=0.009。CTC與乳腺癌分子分型無相關性,P0.05,與腫瘤原發(fā)灶分期、區(qū)域淋巴結(jié)轉(zhuǎn)移、激素受體狀況及HER2表達均無相關性,P0.05。但與是否發(fā)生遠處轉(zhuǎn)移密切相關,χ~2=5.863,P=0.015。結(jié)論 CTC與乳腺癌分子分型無明顯相關,但與患者臨床分期、是否發(fā)生轉(zhuǎn)移密切相關,檢測CTC有利于更準確的評估病情。
[Abstract]:Objective as a new method, the detection of circulating tumor cells has a broad application prospect in clinical practice.In this study, the negative enrichment method was used to detect the level of CTC in peripheral blood of breast cancer patients, and to explore the correlation between the level of CTC and its molecular typing and clinical features.Methods A total of 152 breast cancer patients with CTC were selected from the first people's Hospital of Lianyungang City, 2015-08-2016-02-20. The clinical data were collected and 29 patients with benign breast diseases were selected as the control group.Immunofluorescence in situ hybridization in situ hybridization (fish) was used to detect the CTCs of CD45% CEP- / DAPI-, and the molecular typing and clinical characteristics of breast cancer were analyzed by using immunomagnetic bead negative enrichment and immunofluorescence in situ hybridization (fish) technique.Results the sensitivity and specificity of different cut points were compared by ROC curve analysis in 152 patients with breast cancer and 29 patients with benign breast diseases.The highest Youden index is 0.390, and the corresponding point is 鈮,
