川東北地區(qū)初診食管癌患者營養(yǎng)狀況及生活質(zhì)量調(diào)查分析

【摘要】:目的:了解川東北初診食管癌患者營養(yǎng)狀況及生活質(zhì)量基本情況,分析相關(guān)影響因素,為臨床診治提供參考。方法:收集川東北地區(qū)經(jīng)病理學(xué)檢查確診的初診食管癌病例,利用人體測量指標(biāo)及相關(guān)實驗室指標(biāo)評估營養(yǎng)狀況,采用歐洲癌癥病人生命質(zhì)量核心量表(EORTC QLQ-C30 V3.0中文版)、食管癌專用量表(EORTC QLQ-OES18)評估生活質(zhì)量,采用SPSS單因素及多因素分析初診食管癌患者營養(yǎng)狀況及生活質(zhì)量的影響因素。結(jié)果:收集川東北地區(qū)經(jīng)病理學(xué)檢查確診的初診食管癌病例481例,按體質(zhì)指數(shù)、血清白蛋白、血清前白蛋白、轉(zhuǎn)鐵蛋白、淋巴細(xì)胞總數(shù)、肱三頭肌皮褶厚度作為評價指標(biāo),營養(yǎng)不足發(fā)生率分別為11%、10%、90.9%、84.4%、80.9%、56.5%。以體質(zhì)指數(shù)為判定指標(biāo),單因素分析顯示,不同C反應(yīng)蛋白(C-reactive protein,CRP)水平、病變長度5cm、梗阻程度與食管癌患者營養(yǎng)不足有關(guān)(P0.05),Logistic回歸分析顯示,CRP水平及梗阻程度是食管癌患者營養(yǎng)不足的影響因素。初診食管癌患者的生活質(zhì)量評分較低,總體健康狀況平均64.30±14.33分,功能領(lǐng)域得分從低到高前三位依次是社會功能、情緒功能、角色功能。單因素分析顯示,存在營養(yǎng)不足的患者在總體健康狀況、氣促、腹瀉、咽口水、咳嗽、言語、梗阻方面與非營養(yǎng)不足患者相比差異有顯著性(P0.05),營養(yǎng)不足患者生活質(zhì)量較差。結(jié)論:川東北地區(qū)初診食管癌患者營養(yǎng)不足發(fā)生率較高,CRP水平及梗阻程度是影響營養(yǎng)不足的主要因素;其總體生活質(zhì)量偏低,營養(yǎng)不足是生活質(zhì)量較差的重要影響因素,臨床應(yīng)充分重視患者的營養(yǎng)狀況。
[Abstract]:Objective: to investigate the nutritional status and quality of life (QOL) of newly diagnosed esophageal cancer patients in Northeast Sichuan, and to analyze the related factors and provide reference for clinical diagnosis and treatment.Methods: the first diagnosed esophageal cancer cases in Northeast Sichuan were collected and the nutritional status was evaluated by anthropometric and related laboratory indexes.The quality of life (QOL) was evaluated by EORTC QLQ-C30 V3.0 and EORTC QLQ-OES18. The factors affecting nutritional status and quality of life of newly diagnosed esophageal cancer patients were analyzed by SPSS single factor and multivariate analysis.Results: 481 cases of esophageal cancer diagnosed by pathology in Northeast Sichuan were collected. Body mass index (BMI), serum albumin (SA), serum prealbumin (SA), transferrin, total lymphocyte count and triceps skinfold thickness were used as evaluation indexes.The incidence of malnutrition was 11100.90% and 84.4%, respectively.The single factor analysis showed that different C-reactive protein (CRP) levels were determined by body mass index (BMI).The length of lesion was 5 cm and the degree of obstruction was related to nutritional deficiency of esophageal cancer patients. Logistic regression analysis showed that CRP level and degree of obstruction were the influencing factors of nutritional deficiency in esophageal cancer patients.The quality of life (QOL) score of newly diagnosed esophageal cancer patients was lower, the average health status of the patients was 64.30 鹵14.33. The social function, emotional function and role function were the first three points in the functional field from low to high.Univariate analysis showed that there were significant differences in the overall health status, shortness of breath, diarrhea, pharyngeal saliva, cough, speech and obstruction between the patients with malnutrition and those with non-nutritional deficiency (P 0.05), and the quality of life of the patients with malnutrition was poor.Conclusion: the high level of CRP and the degree of obstruction are the main factors affecting the nutritional deficiency in the newly diagnosed esophageal cancer patients in Northeast Sichuan, and the overall quality of life is low, and the deficiency of nutrition is the important factor affecting the poor quality of life.The nutritional status of patients should be paid more attention to in clinic.
