【摘要】：目的:對(duì)課題組篩選出的食管鱗癌組織中差異蛋白HSP27,SM22,TAGLN2和P38MAPK進(jìn)行驗(yàn)證,并結(jié)合臨床病理參數(shù)篩選可能的分子標(biāo)記物。方法:運(yùn)用Western Blot對(duì)28對(duì)新鮮食管鱗癌組織中的HSP27,SM22,TAGLN2和P38MAPK表達(dá)情況進(jìn)行分析;再運(yùn)用免疫組織化學(xué)技術(shù)對(duì)162例石蠟包埋食管鱗癌組織及50例食管粘膜正常組織中上述蛋白表達(dá)情況進(jìn)行分析;選取部分石蠟包埋組織病例資料進(jìn)行隨訪;結(jié)合臨床病理參數(shù)進(jìn)行數(shù)據(jù)統(tǒng)計(jì),運(yùn)用ROC曲線評(píng)估各蛋白診斷價(jià)值,運(yùn)用Cox回歸模型分析與食管鱗癌預(yù)后相關(guān)因素。結(jié)果:(1)Western Blot發(fā)現(xiàn)HSP27,SM22,TAGLN2和P38MAPK在食管鱗狀細(xì)胞癌腫瘤組織中的表達(dá)升高(P0.05);HSP27,SM22在食管鱗狀細(xì)胞癌淋巴結(jié)轉(zhuǎn)移組表達(dá)升高(P0.05)。(2)免疫組織化學(xué)發(fā)現(xiàn)上述蛋白在石蠟包埋癌組織中的表達(dá)陽性率高于正常粘膜組織(P0.001);臨床病理參數(shù)分析發(fā)現(xiàn)HSP27,SM22,TAGLN2表達(dá)陽性與食管癌淋巴結(jié)轉(zhuǎn)移存在相關(guān)性(P0.05);SM22,TAGLN2,P38MAPK表達(dá)陽性與食管癌浸潤(rùn)深度存在相關(guān)性(P0.05);計(jì)算ROC曲線下面積發(fā)現(xiàn)TAGLN2有較高診斷價(jià)值(AUC=0.902,P0.001),SM22,HSP27,P38MAPK有一定的診斷價(jià)值。(3)通過Cox回歸模型生存分析發(fā)現(xiàn)淋巴結(jié)轉(zhuǎn)移、浸潤(rùn)深度、HSP27表達(dá)與患者生存情況存在相關(guān)性,差異有統(tǒng)計(jì)學(xué)意義(P0.05),而其它因素尚不能認(rèn)為與患者遠(yuǎn)期生存存在相關(guān)性(P0.05)。結(jié)論:HSP27,SM22,TAGLN2,P38MAPK是食管鱗狀細(xì)胞癌發(fā)生發(fā)展過程中的重要變化分子,可能對(duì)食管鱗狀細(xì)胞癌惡性程度判斷有一定的指導(dǎo)意義;TAGLN2作為食管鱗狀細(xì)胞癌診斷的分子標(biāo)記物有較高診斷價(jià)值;HSP27與食管鱗癌患者預(yù)后存在相關(guān)性,可以作為預(yù)后評(píng)估的分子標(biāo)志物。
[Abstract]:Objective: to verify the differential protein HSP27SM22SM22 TAGLN2 and P38MAPK in esophageal squamous cell carcinoma. Methods: Western Blot was used to analyze the expression of Western Blot in 28 pairs of fresh esophageal squamous cell carcinoma tissues. The expression of these proteins in 162 paraffin-embedded esophageal squamous cell carcinoma tissues and 50 normal esophageal mucosa tissues were analyzed by immunohistochemical technique, and some paraffin embedded tissues were selected for follow-up. The ROC curve was used to evaluate the diagnostic value of each protein. Cox regression model was used to analyze the prognostic factors of esophageal squamous cell carcinoma. Results the expression of HSP27 SM22, TAGLN2 and P38MAPK in esophageal squamous cell carcinoma tumor tissues was increased by Blot. The positive rate of the above protein expression in paraffin embedded carcinoma tissue was higher than that in the normal mucosa tissue P0.001.The clinicopathologic parameters analysis showed that the expression of HSP27 SMM22 / TAGLN2 was correlated with lymph node metastasis of esophageal carcinoma. The area under the ROC curve was found to be of high diagnostic value by calculating the area under the ROC curve. There was a certain diagnostic value of TAGLN2 in SM22HSP27P38MAPK). Lymph node metastasis was found by survival analysis of Cox regression model. The expression of HSP27 was correlated with the survival of the patients. The difference was statistically significant (P 0.05), but other factors could not be considered to be related to the long-term survival of the patients. Conclusion: the proportion of HSP27, SM22, TAGLN2 and P38 MAPK is an important variable molecule in the carcinogenesis and development of esophageal squamous cell carcinoma. TAGLN2 may be a molecular marker for the diagnosis of esophageal squamous cell carcinoma. HSP27 may be associated with the prognosis of esophageal squamous cell carcinoma. It can be used as a molecular marker for prognostic evaluation.
