星形細胞上調(diào)基因-1與鈣黏附蛋白-E在食管癌中表達的相關性及其意義

【摘要】:背景食管癌是我國常見消化道惡性腫瘤,由于早期癥狀不典型,多數(shù)患者發(fā)現(xiàn)時已屬晚期,因此具有較高的死亡率,居全國各類惡性腫瘤的第4位,其中深度浸潤和轉(zhuǎn)移是主要的死因。目前對于食管癌的治療除傳統(tǒng)的外科手術、放化療治療外,隨著人們對腫瘤發(fā)病的分子水平的研究不斷加深,針對新分子靶點的分子靶向治療成為食管癌治療的新方向。星形細胞上調(diào)基因-1(astrocyte elevated gene-1,AEG-1)是新近發(fā)現(xiàn)的癌基因。研究發(fā)現(xiàn)AEG-1在多種腫瘤(如乳腺癌、神經(jīng)膠質(zhì)瘤、前列腺癌、胃癌等)中存在高表達,并與腫瘤的進展、轉(zhuǎn)移與侵襲、不良預后密切相關。鈣黏附蛋白-E(E-cadherin,E-ca)是表達于正常上皮細胞表面的黏附分子,介導上皮細胞間的黏附性。E-cadherin在多種腫瘤中存在明顯表達減少甚至缺失,與腫瘤的轉(zhuǎn)移與侵襲、不良預后密切相關。然而兩者在食管癌中表達的相關性并未見報道。該研究擬通過免疫組織化學和Western blot檢測食管癌及相應癌旁食管黏膜中AEG-1和E-ca表達水平,并分析兩者與臨床病理特征的關系,探討兩者在食管癌中表達的相關性及其意義。目的研究AEG-1和E-ca在食管癌中的表達的相關性,并分析與臨床病理特征的關系。初步探討兩者在食管癌浸潤、轉(zhuǎn)移中的作用。方法采用免疫組化方法和Western blot兩種方法檢測58例食管癌組織及其相應的癌旁組織中AEG-1和鈣黏附蛋白-E的表達;并收集患者的完整病理資料。分析兩者與性別、年齡、腫瘤大小、分化程度、淋巴結(jié)轉(zhuǎn)移、臨床分期之間的相關性;應用SPSS19.0統(tǒng)計軟件,計數(shù)資料采用χ2檢驗,相關性采用Spearman相關分析,P0.05為差異有統(tǒng)計學意義。結(jié)果1.在58例食管癌組織中AEG-1表達的陽性率82.7%明顯高于相應的癌旁組織的51.7%(P0.05),且表達陽性率與性別、年齡、腫瘤的部位、腫瘤病理類型不相關(P0.05),而與腫瘤分化程度、浸潤深度、臨床分期、淋巴結(jié)轉(zhuǎn)移相關(P0.05)。2.在58例食管癌組織中鈣黏附蛋白-E的陽性率為31.0%,明顯低于相應癌旁組織的94.8%(P0.05),且表達陽性率與性別、年齡、腫瘤的部位、腫瘤病理類型不相關(P0.05),而與腫瘤分化程度、浸潤深度、臨床分期、淋巴結(jié)轉(zhuǎn)移相關(P0.05)。3.食管癌中AEG-1與E-ca的表達呈明顯的負相關(r=-0.483,P0.05)。結(jié)論1.AEG-1和E-ca共同參與食管癌的侵潤轉(zhuǎn)移過程,聯(lián)合檢測AEG-1和E-ca是判斷食管癌患者轉(zhuǎn)移潛能和預后的重要因素。2.AEG-1可能介導食管癌中E-ca的表達下調(diào)。
[Abstract]:Background esophageal cancer is a common malignant tumor of digestive tract in China. Because of the atypical early symptoms, most of the patients were found to be advanced, so they have a high mortality rate, ranking fourth among all kinds of malignant tumors in China. Among them, deep infiltration and metastasis are the main causes of death. At present, the treatment of esophageal cancer, in addition to traditional surgery, radiotherapy and chemotherapy, with the deepening of the study of the molecular level of tumor, Molecular targeting therapy for new molecular targets has become a new direction in the treatment of esophageal cancer. The astrocyte up-regulation gene elevated gene-1 AEG-1 is a newly discovered oncogene. AEG-1 has been found in many tumors (such as breast cancer, glioma, prostate cancer). High expression was found in gastric cancer, and was closely related to tumor progression, metastasis, invasion and poor prognosis. Calcitonin E-cadherin E-cac is a adhesion molecule expressed on the surface of normal epithelial cells. The expression of E-cadherin, mediated by adhesion between epithelial cells, is reduced or even absent in many kinds of tumors, and it is associated with the metastasis and invasion of tumor. The expression of AEG-1 and E-ca in esophageal carcinoma and its adjacent esophageal mucosa were detected by immunohistochemistry and Western blot. To study the relationship between the expression of AEG-1 and E-ca in esophageal carcinoma, and the relationship between them and clinicopathological features, and to explore the relationship between the expression of AEG-1 and E-ca in esophageal carcinoma. The relationship between them and clinicopathological features was analyzed. Methods Immunohistochemical method and Western blot were used to detect the expression of AEG-1 and cadherin E in 58 cases of esophageal carcinoma and their adjacent tissues, and the complete pathological data of the patients were collected. The correlation between age, tumor size, differentiation degree, lymph node metastasis, clinical stage, SPSS19.0 statistical software, count data by 蠂 2 test, Results 1. The positive rate of AEG-1 expression in 58 cases of esophageal carcinoma was significantly higher than that of the corresponding adjacent tissues (51.7%), and the positive rate of AEG-1 expression was significantly higher than that of the corresponding adjacent tissues (51.7%), and the positive rate was related to sex, age and location of tumor. 2. The pathological type of the tumor was not related to P0.05, but to the degree of differentiation, depth of invasion and clinical stage. The positive rate of cadherin E in 58 cases of esophageal carcinoma was 31.0, which was significantly lower than that of the corresponding adjacent tissues (P 0.05), and the positive rate was related to sex, age and location of tumor. The expression of AEG-1 and E-ca in esophageal carcinoma was negatively correlated with the degree of differentiation, depth of invasion, clinical stage and lymph node metastasis. Conclusion 1.AEG-1 and E-ca are involved in the process of invasion and metastasis of esophageal carcinoma. Combined detection of AEG-1 and E-ca is an important factor in judging metastasis potential and prognosis of esophageal cancer patients. 2. AEG-1 may mediate the down-regulation of E-ca expression in esophageal carcinoma.
