【摘要】：目的:通過(guò)高效液相色譜-質(zhì)譜聯(lián)用技術(shù)(High performance liquid chromatography-mass spectrometry,HPLC-MS)比較早期胃癌組、慢性淺表性胃炎組、正常人群組血清和尿液小分子代謝產(chǎn)物的差異,尋找在早期胃癌診斷中潛在的代謝產(chǎn)物。方法:在胃癌高發(fā)區(qū)廣西壯族自治區(qū)百色市收集早期胃癌組16例、慢性淺表性胃炎患者20例、正常組20例的血清和尿液標(biāo)本。運(yùn)用高效液相色譜-質(zhì)譜聯(lián)用技術(shù)對(duì)各組樣品進(jìn)行檢測(cè),再采用主成分分析方法(Principal component analysis,PCA)、偏最小二乘判別法(Partial least squares discriminant analysis,PLS-DA)、t檢驗(yàn)統(tǒng)計(jì)分析得到早期胃癌組與其余兩組的差異代謝物。最后將找到的差異代謝物通過(guò)人類(lèi)代謝組數(shù)據(jù)庫(kù)(Human metabolic group database,HMDB)檢索鑒定。結(jié)果:對(duì)比正常人,早期胃癌患者血清中共篩選鑒定出9種(二氫膽固醇、膽固醇、酪氨酸、乳酸、酪胺、3-羥基丙酸、丙酮酸、丙氨酸、溶血磷脂酰膽堿(18:0)),尿液中共篩選鑒定出8種(十六烷酸、乳酸、丙酮醛、膽固醇、纈氨酸、甘油、肌醇、天冬氨酸)對(duì)分類(lèi)有顯著貢獻(xiàn)的代謝標(biāo)志物。對(duì)比慢性淺表性胃炎患者,早期胃癌患者血清中共篩選鑒定出9種(L-纈氨酸、膽固醇、L-肉堿、鳥(niǎo)氨酸、二十二碳六烯酸、丙酮醛、溶血磷脂酰膽堿(16:0)、琥珀酸、檸檬酸),尿液中共篩選鑒定出7種(琥珀酸、丙酸、絲氨酸、L-異亮氨酸、膽固醇、2-羥基丁酸、甲硫氨酸)對(duì)分類(lèi)有顯著貢獻(xiàn)的代謝標(biāo)志物。這些代謝物的變化提示早期胃癌存在糖類(lèi)、脂類(lèi)、氨基酸代謝異常。結(jié)論:基于HPLC-MS的代謝組學(xué)分析方法能夠有效的區(qū)分早期胃癌與慢性淺表性胃炎、正常人個(gè)體,對(duì)早期胃癌的診斷具有潛在的價(jià)值。
[Abstract]:Objective: to compare the differences of small molecule metabolites in serum and urine between early gastric cancer group, chronic superficial gastritis group and normal group by high performance liquid chromatography-mass spectrometrymetric chromatography-mass (HPLC-MS), high performance liquid chromatography-mass spectrometry (HPLC / MS). Methods: 16 cases of early gastric cancer and 20 cases of chronic superficial gastritis were collected in Baise City of Guangxi Zhuang Autonomous region in high incidence area of gastric cancer. The serum and urine samples of 20 normal subjects were detected by high performance liquid chromatography-mass spectrometry (HPLC / MS). Then the principal component analysis (PCA) method was used to obtain the differential metabolites between the early gastric cancer group and the other two groups by partial least squares discriminant analysis and partial least squares discriminant method. Finally, the difference metabolites were found through the number of human metabolic groups. Human metabolic group database (HMDBs). Results: compared with normal people, A total of 9 species (dihydrocholesterol, cholesterol, tyrosine, lactic acid, tyramine 3-hydroxypropionic acid, pyruvate, alanine, lysophosphatidylcholine 18: 0) were identified in serum of patients with early gastric cancer. Lactic acid, acetone aldehyde, cholesterol, valine, glycerol, inositol, aspartic acid) are the metabolic markers that contribute significantly to the classification. A total of 7 species (succinic acid, propionic acid, serine L-isoleucine, cholesterol 2-hydroxybutyric acid) were screened and identified by cholesterol L-carnitine, ornithine, 22 carbohexaenoic acid, pyruval, lysophosphatidylcholine 16: 0, succinic acid, citric acid, urine. The changes in these metabolites suggest that early gastric cancer contains carbohydrates, lipids, Conclusion: HPLC-MS based metabonomics analysis method can effectively distinguish early gastric cancer from chronic superficial gastritis, which has potential value in the diagnosis of early gastric cancer.
