外周血淋巴細(xì)胞亞群檢測在肺癌患者臨床評估及輔助治療中的意義

【摘要】:目的:通過觀察外周血淋巴細(xì)胞亞群在肺癌患者的表達(dá)情況,探討肺癌患者的免疫功能是否存在一定的變化規(guī)律,從而更好地評估肺癌患者的病情及預(yù)后,以期為其輔助治療提供更多的參考。方法:選擇2015年8月至2016年12月于我院確診并進(jìn)行2個周期以上(包括2個周期)化療的肺癌患者63例,同期體檢健康者30例,前者作為觀察組,后者作為對照組,借助流式細(xì)胞術(shù),測定觀察組化療前及完成第2個周期化療后的外周血CD3+T淋巴細(xì)胞、CD4+T淋巴細(xì)胞、CD8+T淋巴細(xì)胞和NK細(xì)胞的比例,并測定上述指標(biāo)在對照組的表達(dá)情況,比較觀察組和對照組之間、觀察組中不同病理類型之間、不同臨床分期之間以及第一個周期化療前和第二個周期化療后上述各指標(biāo)的差異。所有數(shù)據(jù)以均數(shù)±標(biāo)準(zhǔn)差(?x±s)的形式表示,用SPSS 20.0統(tǒng)計(jì)軟件(中文版)處理所得數(shù)據(jù)。以P=0.05作為檢驗(yàn)水準(zhǔn)判斷差異是否具有統(tǒng)計(jì)學(xué)意義。結(jié)果:1.觀察組外周血CD3+T淋巴細(xì)胞、CD4+T淋巴細(xì)胞和NK細(xì)胞比例較對照組降低,CD8+T淋巴細(xì)胞比例較對照組有所升高。2.不同病理類型的肺癌患者,其外周血CD3+T淋巴細(xì)胞、CD4+T淋巴細(xì)胞、CD8+T淋巴細(xì)胞和NK細(xì)胞比例無顯著差異。3.不同分期的肺癌患者,分期越晚,其外周血CD3+T淋巴細(xì)胞、CD4+T淋巴細(xì)胞和NK細(xì)胞比例越低,CD8+T淋巴細(xì)胞比例越高。4.2個周期化療后PS評分≤2的肺癌患者,其外周血CD3+T淋巴細(xì)胞、CD4+T淋巴細(xì)胞和NK細(xì)胞比例較化療前有所升高,CD8+T淋巴細(xì)胞比例有所回落;2個周期化療后PS評分2的肺癌患者,其CD3+T淋巴細(xì)胞、CD4+T淋巴細(xì)胞、CD8+T淋巴細(xì)胞和NK細(xì)胞比例較化療前變化不顯著。結(jié)論:多數(shù)肺癌患者出現(xiàn)不同程度的免疫功能下降,晚期肺癌患者的免疫功能較早期肺癌患者低,不同病理類型肺癌患者的免疫功能無顯著差異;規(guī)范的化療可以在一定程度上促進(jìn)肺癌患者免疫功能的恢復(fù);治療肺癌時應(yīng)考慮給予適當(dāng)?shù)拿庖哒{(diào)節(jié)治療作為輔助治療。
[Abstract]:Objective: to observe the expression of peripheral blood lymphocyte subsets in patients with lung cancer, and to explore whether the immune function of lung cancer patients has a certain change rule, so as to better evaluate the condition and prognosis of lung cancer patients. Methods: from August 2015 to December 2016, 63 patients with lung cancer were diagnosed in our hospital and received 2 cycles of chemotherapy (including 2 cycles). The former served as the observation group and the latter as the control group. Flow cytometry was used to measure the percentage of CD4, CD8 and NK cells in peripheral blood CD3 T lymphocytes before and after the second cycle of chemotherapy in the observation group. The expression of the above indexes in the control group was measured, and the pathological types were compared between the observation group and the control group, and between the observation group and the control group. The differences of the above indexes between different clinical stages, before and after the first cycle chemotherapy and after the second cycle chemotherapy. In the form of x 鹵s, SPSS 20.0 statistical software (Chinese version) was used to process the data. The difference was statistically significant using P0. 05 as the test level. Results: 1. The proportion of CD4 T lymphocytes and NK cells in peripheral blood CD3 T lymphocytes in the observation group was higher than that in the control group. The percentage of CD8 T lymphocytes decreased in the radiation group was higher than that in the control group. 2. Patients with different pathological types of lung cancer, There was no significant difference in the proportion of CD8 T lymphocytes and NK cells between CD4 T lymphocytes and NK cells in peripheral blood CD3 T lymphocytes. 3. The later the stage was, the later the stage was in patients with different stages of lung cancer.
