【摘要】：目的整合我國肝癌經(jīng)濟(jì)負(fù)擔(dān)證據(jù),為今后研究提供基線數(shù)據(jù)和方法參考。方法系統(tǒng)檢索Pub Med、中國知網(wǎng)和萬方數(shù)據(jù)庫1996-2015年間發(fā)表文獻(xiàn),描述納入研究基本特征并分析經(jīng)濟(jì)負(fù)擔(dān)結(jié)果,費(fèi)用貼現(xiàn)至2013年值,采用美國衛(wèi)生保健質(zhì)量和研究機(jī)構(gòu)(agency for health care research and quality,AHRQ)推薦的量表開展質(zhì)量評價(jià)。結(jié)果最終納入32篇文獻(xiàn)。2篇基于人群的研究,報(bào)道的經(jīng)濟(jì)負(fù)擔(dān)分別為37.8億元(省級,直接和間接)和6.0億元(市級,間接)。30篇基于個(gè)體的研究均報(bào)道了直接醫(yī)療費(fèi)用,例均費(fèi)用(21篇)年度中位數(shù)為11 663元(3 500~70 567元),次均費(fèi)用(9篇)年度中位數(shù)為19 202元(7 005~41 118元),日均費(fèi)用(13篇)年度中位數(shù)為865元(409~1 275元);20年間三種指標(biāo)費(fèi)用均呈緩慢上升趨勢;細(xì)化構(gòu)成藥品費(fèi)居多(約56.6%);5篇基于個(gè)體的研究報(bào)道了間接費(fèi)用,中位數(shù)為73 440元(35 815~166 967元)。納入研究質(zhì)量一般。結(jié)論肝癌在我國人群層面經(jīng)濟(jì)負(fù)擔(dān)研究有限。不同地區(qū)的肝癌經(jīng)濟(jì)負(fù)擔(dān)研究力度與其疾病負(fù)擔(dān)不相匹配。例均費(fèi)用數(shù)據(jù)報(bào)道較多,但結(jié)果解釋應(yīng)謹(jǐn)慎。此外,肝癌所致間接經(jīng)濟(jì)負(fù)擔(dān)不容忽視。
[Abstract]:Objective to integrate the evidence of the economic burden of liver cancer in China, and to provide baseline data and methodological references for future research. Methods Pub Med, China knowledge Network and Wanfang Database were searched systematically from 1996 to 2015. Description includes the basic characteristics of the study and analyses the results of economic burden. The cost is discounted to 2013. The quality evaluation was carried out by using the scale recommended by for health care research and quality Agency. Results 32 articles were included in this study. The economic burden of the report was 3.78 billion yuan (provincial level). Both direct and indirect) and 600 million yuan (municipal, indirect and 30 individual based studies) all reported direct medical costs, The median annual average cost per case is $11,663 / 3 500 / $70,567, the average annual average cost is $19,202 / 7 005 / 41 / 118, and the daily average cost is 13) the median annual median is $865 / $409/ $1,275 / 20 / 20. A slow upward trend; The breakdown of the composition of drug costs (about 56. 6%) reported indirect costs in 5 individual-based studies. The median value was 73 440 yuan / 35 815 / 166 / 967 yuan. Conclusion the study on the economic burden of liver cancer at the population level in China is limited. The study on the economic burden of liver cancer in different areas does not match its disease burden. There are more reports with data, But the results should be interpreted with caution. In addition, the indirect economic burden caused by liver cancer should not be ignored.
