【摘要】：目的探討Survivin和Ki-67在橋本氏甲狀腺炎(Hashimoto's Thyroiditis, HT)、甲狀腺乳頭狀癌(Papillary Thyroid Carcinoma, PTC)、及橋本氏甲狀腺炎合并甲狀腺乳頭狀癌中的表達(dá)及臨床病理意義。方法采用免疫組化SP法檢測(cè)140例甲狀腺標(biāo)本中Survivi n和Ki-67蛋白表達(dá)情況,其中甲狀腺腺瘤20例、HT標(biāo)本40例、PTC標(biāo)本40例、HT合并PTC標(biāo)本40例。結(jié)果1、Survivin蛋白表達(dá)在甲狀腺腺瘤組織、HT、PTC; HT合并PTC組織中的陽(yáng)性率分別為10%(2/20)、42.5%(17/40)、77.5%(31/40)、72.5%(29/40)；HT組(χ2=6.508,p=0.011)、PTC組(χ2=24.545,p=0.000)及HT合并PTC組(χ2=20.857,p=0.000)中Survivin蛋白表達(dá)陽(yáng)性率均明顯高于甲狀腺腺瘤組,HT合并PTC中Survivin蛋白的表達(dá)也顯著高于HT組(χ2=7.366,p=0.007),而HT合并PTC組與PTC組比較差異無統(tǒng)計(jì)學(xué)意義(p0.05)。2、臨床資料分析顯示Survivin蛋白在HT合并PTC中的表達(dá)與臨床分期(χ2=4.477,p=0.03)及淋巴轉(zhuǎn)移(χ2=6.930,p=0.008)相關(guān),而與年齡(χ2=0.189,p=0.664)及性別(χ2=0.005,p=0.944)沒有明顯關(guān)聯(lián)。3、Ki-67蛋白表達(dá)在甲狀腺腺瘤組織、HT、PTC、HT合并PTC組織中的陽(yáng)性率分別為15%(3/20)、57.5%(23/40)、67.5%(27/40)、65%(26/40)；HT組(χ2=9.808,p=0.002)、PTC組(χ2=14.700,p=0.000)及HT合并PTC組(χ2=13.348,p=0.000)中Ki-67蛋白表達(dá)陽(yáng)性率均顯著高于甲狀腺腺瘤組織,HT合并PTC組中Ki-67蛋白表達(dá)陽(yáng)性率與HT組(p0.05)及PTC組(p0.05)比較差異無統(tǒng)計(jì)學(xué)意義。4、臨床資料分析顯示,HT合并PTC組中Ki-67蛋白的表達(dá)與臨床分期(x2=4.062,p=0.044)及淋巴轉(zhuǎn)移(χ2=7.016,p=0.008)相關(guān),而與年齡(χ2=0.101,p=0.75)及性另(x2=0.23,p=0.631)沒有明顯關(guān)聯(lián)。5、在HT合并PTC組織中,Survivin和Ki-67的陽(yáng)性表達(dá)呈正相關(guān)(r=0.146,p=0.007)。結(jié)論HT與PTC之間關(guān)系密切,HT具有一定的癌變傾向,HT可能為HT合并PTC的癌前病變,而Survivin和Ki-67的異常高表達(dá)可能在HT合并PTC的發(fā)生、發(fā)展、侵襲及遠(yuǎn)處轉(zhuǎn)移等過程中起著重要的作用。
[Abstract]:Objective to investigate the expression and clinicopathological significance of Survivin and Ki-67 in Hashimoto's thyroiditis Thyroiditis, Thyroiditis, papillary Thyroid Carcinoma, papillary thyroid carcinoma, and Hashimoto's thyroiditis complicated with thyroid papillary carcinoma. Methods Immunohistochemical SP method was used to detect the expression and clinicopathological significance of Survivin and Ki-67 in patients with thyroid papillary carcinoma. The expression of Survivi n and Ki-67 protein in 140 thyroid specimens. Results 1 the positive rate of survivin protein expression in thyroid adenoma tissues was 102 / 20 / 42.5 / 42.5 / 77.5A = 31 / 4072.5% / 29 / 29 / 40 ~ (40) T (蠂 ~ 26.508p 0.011p0.000), respectively (蠂 ~ 26.508p 0.011p0.000) and HT + PTC (224.857p0.000), respectively. The positive rate of survivin protein expression in thyroid adenoma tissues was 102.20% 42.5% ~ 77.55% (蠂 ~ 2 6.508p0.011p0.000) and that of HT combined with PTC was 102.20% ~ 47.55% respectively (蠂 ~ -6.508p 0.011p0.000), and that of HT combined with PTC was 107.55% (蠂 ~ 26.508p 0.011p0.000). The positive rate of Survivin protein expression in HT + PTC was significantly higher than that in HT + PTC in thyroid adenoma group (蠂 ~ 2 / 7.366p ~ (0.007) P ~ (0.007)), but there was no significant difference between HT + PTC group and PTC group (P < 0.05). The clinical data analysis showed that Survivin protein was not significantly different from that in HT + PTC group. The expression of HT combined with PTC was correlated with clinical stage (蠂 2 / 4.477) and lymphatic metastasis (蠂 2 / 6.930 / P 0.008). However, there was no significant correlation between age (蠂 2 / 0.189) and sex (蠂 2 / 0.005 / P ~ (0.944)). The positive rate of Ki-67 protein expression in thyroid adenoma tissues with PTC was 15 ~ 320% ~ 57.5%, respectively. The positive rate of Ki-67 protein expression in HT group was significantly higher than that in HT group (蠂 ~ 29.808) and HT combined with PTC group (213.348p0.000), respectively. The positive rate of expression of Ki-67 protein was significantly higher in HT group (蠂 29.808) and HT combined with PTC group (213.348p0.000) than in the control group (蠂 29.808) and HT combined with PTC group (213.348p0.000), respectively. The clinical data analysis showed that the expression of Ki-67 protein was correlated with the clinical stage and lymph metastasis (蠂 27.016p0.008). However, there was no significant correlation with age (蠂 ~ 2 + 0.101) and sex (X ~ (2 +) = 0.23 ~ (0.23) P ~ (0.63)). There was a positive correlation between the expression of survivin and Ki-67 in HT with PTC. Conclusion there is a close relationship between HT and PTC. HT may be the precancerous lesion of HT combined with PTC. The abnormal overexpression of Survivin and Ki-67 may play an important role in the occurrence, development, invasion and distant metastasis of HT associated with PTC.
