HLA-DRB1基因多態(tài)性與乳腺癌關(guān)聯(lián)研究的Meta分析

【摘要】:目的:用Meta分析的方法綜合評(píng)價(jià)人類白細(xì)胞抗原基因DRB1座位(human leukocyte antigen DRB1,HLA-DRB1)基因多態(tài)性與乳腺癌的關(guān)系。方法:檢索中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫、萬方數(shù)據(jù)庫、中國知網(wǎng)、維普醫(yī)藥數(shù)據(jù)庫、超星medalink、 MEDLINE、Embase和PubMed數(shù)據(jù)庫,查閱并收集國內(nèi)外公開發(fā)表的有關(guān)HLA—DRB1基因多態(tài)性與乳腺癌關(guān)聯(lián)研究的文獻(xiàn),由2位研究者根據(jù)預(yù)先設(shè)定的納入和排除標(biāo)準(zhǔn)對(duì)收集的文獻(xiàn)進(jìn)行評(píng)價(jià),對(duì)符合納入標(biāo)準(zhǔn)的文獻(xiàn)進(jìn)行信息提取并交叉核對(duì)。應(yīng)用RevMan5.2統(tǒng)計(jì)軟件對(duì)符合條件的研究結(jié)果進(jìn)行Meta分析,首先進(jìn)行異質(zhì)性檢驗(yàn),根據(jù)檢驗(yàn)結(jié)果選擇效應(yīng)模型,最后對(duì)合并結(jié)果進(jìn)行Z檢驗(yàn),P≤≤0.05有統(tǒng)計(jì)學(xué)意義。結(jié)果:篩查后共納入8篇文獻(xiàn),其中1篇為中文文獻(xiàn),其余7篇為英文文獻(xiàn),共8個(gè)研究報(bào)道HLA-DRB1基因多態(tài)性與乳腺癌的關(guān)聯(lián)。根據(jù)Meta分析結(jié)果,乳腺癌病例組與對(duì)照組相比,具有更高的HLA-DRB1*03、HLA-DRB1*09基因型比例(OR=1.52, 95%CI:1.16~1.99,P=0.002;OR=1.45,95%CI:1.02~2.07,P=0.04)。根據(jù)研究對(duì)象的種族,對(duì)白種人的研究進(jìn)行亞組分析,亞組包括4個(gè)研究。結(jié)果是,乳腺癌病例組與對(duì)照組相比,具有更高的HLA-DRB1*03、HLA-DRB1*12基因型比例(OR=1.75,95%CI:1.21~2.54,P=0.003;OR=2.59,95%CI:1.12~5.96,P=0.03)。結(jié)論:HLA-DRB1*03和HLA-DRB1*09基因型可能是乳腺癌發(fā)生中的危險(xiǎn)因素,而對(duì)于白種人HLA-DRB1*03和HLA-DRB1*12基因型可能是乳腺癌發(fā)生中的危險(xiǎn)因素。
[Abstract]:Objective: to evaluate the relationship between HLA-DRB1) polymorphism of human leukocyte antigen gene DRB1 locus and breast cancer by Meta analysis. Weip Medical Database, Superstar Medalink, MEDLINE Embase and PubMed databases were used to review and collect the published literature on the association between HLA-DRB1 gene polymorphism and breast cancer. The collected literature was evaluated by two researchers according to the pre-set inclusion and exclusion criteria, and the information of the documents that met the inclusion criteria was extracted and cross-checked.
