PEDF在食管鱗狀細(xì)胞癌中的表達(dá)及臨床意義

【摘要】:目的:研究色素上皮衍生因子(Pigment epithelium-derived factor,PEDF)在食管鱗狀細(xì)胞癌組織中的表達(dá)及其臨床意義,探討食管鱗狀細(xì)胞癌PEDF蛋白表達(dá)情況與微血管密度(MVD)的相關(guān)性。方法:取2016年2月至2016年10月四川省南充市中心醫(yī)院和川北醫(yī)學(xué)院附屬醫(yī)院心胸外科手術(shù)切除的食管鱗狀細(xì)胞癌組織及周圍正常食管組織64例,所有患者術(shù)前均未接受放療、化療和其他抗腫瘤藥物的干預(yù)。應(yīng)用實(shí)時(shí)熒光定量PCR(real-time quantitative PCR,RT-PCR)檢測(cè)64例食管鱗狀細(xì)胞癌組織和同一患者周圍正常食管組織中PEDF mRNA表達(dá),應(yīng)用免疫組化檢測(cè)64例食管鱗狀細(xì)胞癌組織和同一患者周圍正常食管組織中PEDF蛋白表達(dá)及癌組織中MVD表達(dá)水平。結(jié)果:實(shí)時(shí)熒光定量PCR結(jié)果表明:64對(duì)實(shí)驗(yàn)組織標(biāo)本中PEDF mRNA均有不同強(qiáng)度的表達(dá),其中食管鱗狀細(xì)胞癌組織中PEDF mRNA相對(duì)定量值為0.77,比配對(duì)的周圍正常食管組織(1.04)明顯下降,兩者之間比較有統(tǒng)計(jì)學(xué)差異(P0.05)。免疫組化檢測(cè)結(jié)果表明:(1)PEDF蛋白的表達(dá)情況:64例食管鱗狀細(xì)胞癌組織標(biāo)本中29例PEDF蛋白表達(dá)陽(yáng)性,35例PEDF蛋白表達(dá)陰性,陽(yáng)性表達(dá)率為45.31%,而在正常食管組織標(biāo)本中43例PEDF蛋白表達(dá)陽(yáng)性,21例PEDF蛋白表達(dá)陰性,陽(yáng)性表達(dá)率為67.19%,兩組之間表達(dá)差異有統(tǒng)計(jì)學(xué)意義(P0.05)。食管鱗狀細(xì)胞癌組織中PEDF蛋白表達(dá)與患者的年齡、性別無(wú)明顯相關(guān)(P0.05),而與腫瘤的臨床分期、組織分化程度和淋巴結(jié)轉(zhuǎn)移有關(guān)(P0.05),且腫瘤組織分化程度越低、分期越晚或有淋巴結(jié)轉(zhuǎn)移者,PEDF蛋白表達(dá)越低。(2)食管鱗狀細(xì)胞癌組織中PEDF蛋白表達(dá)與MVD的關(guān)系:29例PEDF表達(dá)陽(yáng)性的食管鱗狀細(xì)胞癌組織MVD均值是38.3793,而35例PEDF表達(dá)陰性的食管鱗狀細(xì)胞癌組織MVD均值是43.4857,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:PEDF在食管鱗狀細(xì)胞癌組織中表達(dá)降低,且其表達(dá)降低對(duì)食管鱗狀細(xì)胞癌預(yù)后的判斷可能有一定的參考價(jià)值。PEDF具有選擇性的抗新生血管生成作用,使它成為抗腫瘤治療的潛力分子,但其有效性、安全性、用藥途徑(包括合適的載體途徑)都需要更多的研究進(jìn)一步證實(shí)。
[Abstract]:Objective: to study the expression and clinical significance of pigment epithelium-derived factor epithelium-derived factor (PEDF) in esophageal squamous cell carcinoma (esophageal squamous cell carcinoma). To investigate the correlation between the expression of PEDF protein and microvessel density (MVD) in esophageal squamous cell carcinoma (SCC). Methods: from February 2016 to October 2016, cardiothoracic surgery was performed at Nanchong Central Hospital of Sichuan Province and affiliated Hospital of North Sichuan Medical College. There were 64 cases of esophageal squamous cell carcinoma and surrounding normal esophageal tissue. All the patients were not treated with radiotherapy, chemotherapy and other anti-tumor drugs before operation. The expression of PEDF mRNA in 64 esophageal squamous cell carcinoma tissues and the normal esophageal tissues around the same patient was detected by real-time quantitative PCR(real-time quantitative rrrrrrrrrrrrrtPCR.The expression of PEDF mRNA in 64 cases of esophageal squamous cell carcinoma and normal esophageal tissues around the same patient was detected. Immunohistochemistry was used to detect the expression of PEDF protein in 64 cases of esophageal squamous cell carcinoma and normal esophageal tissues around the same patient and the expression level of MVD in cancer tissue. Results: the real-time fluorescence quantitative PCR results showed that the expression of PEDF protein in the tissues of 64 cases of esophageal squamous cell carcinoma and the normal esophageal tissues around the same patient. PEDF mRNA were expressed in different intensity. The relative quantitative value of PEDF mRNA in esophageal squamous cell carcinoma was 0.77, which was significantly lower than that in matched normal esophageal tissue (1.04). The expression of PEDF protein in 29 cases of esophageal squamous cell carcinoma was negative in 29 cases of squamous cell carcinoma of esophagus, and the expression of PEDF protein was negative in 35 cases, the results of immunohistochemistry showed that the expression of PEDF protein was negative in 29 cases of esophageal squamous cell carcinoma, and the expression of PEDF protein was negative in 29 cases of squamous cell carcinoma of esophagus. The positive expression rate of PEDF protein was 45.31%, while the positive expression of PEDF protein was negative in 21 cases in 43 normal esophageal tissues. The positive expression rate was 67.19, and the difference between the two groups was statistically significant (P 0.05). There was no significant correlation between the expression of PEDF protein and the age and sex of the patients, but there was no significant correlation between the expression of PEDF protein and the clinical stage of the tumor. The degree of tissue differentiation is related to lymph node metastasis, and the lower the degree of differentiation of tumor tissue is, Relationship between PEDF protein expression and MVD in esophageal squamous Cell carcinoma tissues with late staging or lymph Node Metastasis MVD mean value of MVD in 29 cases of esophageal squamous Cell carcinoma with positive PEDF expression was 38.3793, but negative in 35 cases. The average value of MVD in esophageal squamous cell carcinoma was 43.4857, and the difference was statistically significant (P 0.05). Conclusion the expression of MVD in esophageal squamous cell carcinoma is decreased. The lower expression of PEDF may be useful in judging the prognosis of esophageal squamous cell carcinoma. PEDF has a selective anti-angiogenic effect, which makes it a potential molecule for antitumor therapy, but it is effective and safe. The drug pathway (including the appropriate carrier pathway) needs more research to further confirm.
