【摘要】：目的:通過Meta分析評(píng)估環(huán)氧化酶-2(COX-2)在肝癌中的表達(dá)及其與臨床病理特征間的關(guān)系,以期尋找肝癌診斷、化學(xué)預(yù)防及治療的新型分子靶點(diǎn)。方法:通過檢索CNKI、萬方、Cochrane圖書館、Pub Med等數(shù)據(jù)庫(kù),搜集從建庫(kù)至2016.10國(guó)內(nèi)外公開發(fā)表的有關(guān)COX-2在肝癌中表達(dá)的病例對(duì)照研究,納入采用免疫組化法檢測(cè)COX-2蛋白,并將染色范圍及染色強(qiáng)度相結(jié)合評(píng)估其表達(dá)水平,且NOS評(píng)分量表≥5分的高質(zhì)量研究。應(yīng)用Rev Man5.3進(jìn)行系統(tǒng)評(píng)價(jià)及異質(zhì)性分析,計(jì)算合并OR及95%CI,對(duì)存在異質(zhì)性的研究進(jìn)行亞組分析以探討其來源,應(yīng)用Stata12.0繪制漏斗圖及Egger回歸方程評(píng)估有無發(fā)表偏倚。結(jié)果:共有21篇文獻(xiàn)納入本次系統(tǒng)評(píng)價(jià),Meta分析結(jié)果顯示:COX-2在肝癌組中的表達(dá)高于正常組(OR=15.69,95%CI=9.73~25.32,P0.00001);COX-2在癌旁組中的表達(dá)高于正常組(OR=9.47,95%CI=4.70~19.10,P0.00001);COX-2在高-中分化肝癌組的表達(dá)高于低分化組(OR=1.79,95%CI=1.31~2.46,P=0.0003),差異均具有顯著性;在COX-2與性別、腫瘤大小、TNM分期等其他9項(xiàng)HCC臨床病理特征關(guān)系的比較中,各組間差異并無統(tǒng)計(jì)學(xué)意義。結(jié)論:COX-2在肝癌及癌旁組織中表達(dá)增高,且主要在肝癌發(fā)生的早期階段而非進(jìn)展階段發(fā)揮作用。
[Abstract]:Objective: to evaluate the expression of cyclooxygenase-2 (COX-2) in hepatocellular carcinoma (HCC) and its relationship with clinicopathological features by Meta analysis in order to find out the diagnosis of HCC. Methods: a new molecular target for chemoprevention and therapy. Methods: by searching the database of Med and Pub Med, we collected the case control studies on the expression of COX-2 in liver cancer published from the establishment of the library to the year 2016.10. The expression level of COX-2 protein was evaluated by immunohistochemical method, and the expression level was evaluated by combining the staining range and the staining intensity.
