【摘要】：目的:探討原發(fā)性惡性心包間皮瘤(PPM)誤診情況并分析可能原因。方法:檢索中文數(shù)據(jù)庫,收集中國內(nèi)地近35年報道的、有臨床診斷或影像學記錄的PPM患者診斷及誤診信息。結(jié)果:164例患者中,65例(39.6%)患者報道誤診,且存在誤診為多種疾病的情況(共88例次):71例次(80.7%)臨床誤診,17例次(19.3%)影像學誤診。前者以結(jié)核性心包炎最常見(40例次,56.3%),其次為縮窄性心包炎(17例次,23.9%);后者以縱隔占位最常見(11例次,64.7%),其次為心包囊腫(4例次,23.5%)。結(jié)論:PPM罕見的發(fā)病率及缺乏特異的臨床和影像學表現(xiàn)導致該病早期易誤診,結(jié)核性心包炎是最常被誤診的疾病,反復細胞學、組織病理學檢查結(jié)合免疫組織化學分析有助于明確診斷。
[Abstract]:Objective: to investigate the misdiagnosis of primary malignant pericardial mesothelioma (PPM) and analyze the possible causes. Diagnostic and misdiagnosis information of PPM patients with clinical diagnosis or imaging records. Results 65 out of 164 PPM patients were reported misdiagnosed. Imaging misdiagnosis was found in 17 cases of tuberculous pericarditis (40 cases with tuberculous pericarditis), followed by 17 cases of constrictive pericarditis (23. 9%) and 17 cases of mediastinal pericarditis (23. 9%), which were misdiagnosed as multiple diseases (88 cases, 71 times, 80.7 times), and 17 cases with tuberculous pericarditis, 17 cases with tuberculous pericarditis, and 17 cases with tuberculous pericarditis, respectively. The most common occurrences were 11 cases (64.7%), followed by pericardial cysts (4 cases). Conclusion the rare incidence of PPM and the lack of specific clinical and imaging manifestations lead to early misdiagnosis of the disease. Tuberculous pericarditis is the most frequently misdiagnosed disease.
