【摘要】：目的:探討無(wú)遠(yuǎn)處轉(zhuǎn)移的晚期陰莖癌的手術(shù)療效及影響預(yù)后的因素。方法:分析2007年9月至2015年7月行陰莖切除術(shù)及淋巴結(jié)清掃術(shù)的無(wú)遠(yuǎn)處轉(zhuǎn)移晚期陰莖癌8例患者的臨床資料,患者年齡37~67歲,平均51.1歲,術(shù)后隨訪4~60個(gè)月,平均19.25個(gè)月,分析影響患者預(yù)后的因素及手術(shù)療效。結(jié)果:術(shù)后3例存活至今,另5例死亡,死亡率62.5%,從手術(shù)到死亡時(shí)間4~13個(gè)月,平均9個(gè)月。術(shù)后無(wú)明顯并發(fā)癥,4例大面積缺損患者術(shù)后皮瓣均預(yù)后良好。結(jié)論:無(wú)遠(yuǎn)處轉(zhuǎn)移的晚期陰莖癌患者預(yù)后不佳,淋巴結(jié)轉(zhuǎn)移是影響預(yù)后的重要因素,手術(shù)是一種相對(duì)有效的治療手段,皮瓣可以修復(fù)腫瘤切除后的大面積皮膚缺損,尚無(wú)足夠資料證明綜合療法是否有效。
[Abstract]:Objective: to investigate the operative effect and prognostic factors of advanced penile carcinoma without distant metastasis. Methods: from September 2007 to July 2015, 8 cases of advanced penile carcinoma without distant metastasis underwent penectomy and lymph node dissection were analyzed. Clinical data of patients, The age of the patients was 37 ~ 67 years (mean 51.1 years). The follow-up period was 4 ~ 60 months (mean 19.25 months). The factors affecting the prognosis and the outcome of the operation were analyzed. Results: three patients survived until now, and five died, the mortality rate was 62.5 months, the time from operation to death was 413 months. The average was 9 months. The postoperative flap had a good prognosis in 4 patients with large area defect without obvious complications. Conclusion: the prognosis of the patients with advanced penile carcinoma without distant metastasis is not good. Lymph node metastasis is an important factor affecting the prognosis. Operation is a relatively effective treatment, flap can repair the large area of skin defect after tumor resection, but there is not enough data to prove whether the combined therapy is effective or not.
