【摘要】:目的:研究論證磷酸丙糖異構(gòu)酶(triose phosphate isomerase TPI)在人肺腺癌組織與配對的癌旁正常肺組織中的差異表達,以及在術(shù)前血清和術(shù)后血清樣本中的差異表達,并探討該酶在血清中與組織中的表達程度有無相關(guān)性,即是否可以用血清中檢測代替組織中的檢測,從而更方便有效地為肺腺癌的診斷和預(yù)后做出評價。方法:(1)取44例新鮮人肺腺癌組織及其配對的癌旁正常肺組織來自湖南省腫瘤醫(yī)院胸外一科的未經(jīng)放療、化療的肺癌手術(shù)切除的肺葉標本,并經(jīng)湖南省腫瘤醫(yī)院病理診斷確診為肺腺癌(不含其它組織癌變成分);(2)取24例肺良性疾病患者對照組,標本來源為湖南省腫瘤醫(yī)院胸外一科手術(shù)切除的肺組織,包括支氣管肺炎、炎性假瘤、肺結(jié)核等,所有肺良性疾病患者均無腫瘤病史;(3)抽取上述44例人肺腺癌患者以及24例肺良性疾病患者術(shù)前1天的血液標本10ml及術(shù)后第7天的血液標本10ml。統(tǒng)計各組TPI總體均數(shù)并進行統(tǒng)計學(xué)分析,血清ELISA方法采用Wilcoxon秩和檢驗,免疫組化結(jié)果采用秩和檢驗。結(jié)果:ELISA結(jié)果分析:(1)術(shù)前肺部良性病變組(A1)與術(shù)后肺部良性病變組(A2)相比較,使用Wilcoxon秩和檢驗,Z=0.517,雙側(cè)檢驗P=0.605,P0.05,無統(tǒng)計學(xué)意義;(2)術(shù)前肺腺癌組(B1)與術(shù)后肺腺癌組(B2)相比較,使用Wilcoxon秩和檢驗,Z=1.228,雙側(cè)檢驗P=0.219,P0.05,無統(tǒng)計學(xué)意義;(3)肺良性病變組與肺腺癌組的下降值相比較,使用Wilcoxon秩和檢驗,Mann-Whitney U=430;Wilcoxon W=706;Z=0.270,雙側(cè)檢驗P=0.787,P0.05,無統(tǒng)計學(xué)意義;(4)兩者組間相比較,即A1與B1相比較,使用Wilcoxon秩和檢驗,Z=0.274,雙側(cè)檢驗P=0.784,P0.05,無統(tǒng)計學(xué)意義。免疫組化統(tǒng)計結(jié)果分析:(1)磷酸丙糖異構(gòu)酶在良性病變組織、肺腺癌組織和與其配對的正常肺組織表達有顯著差異性,P0.01;(2)磷酸丙糖異構(gòu)酶在配對的正常肺組織和肺良性病變組織之間沒有統(tǒng)計學(xué)意義,P0.05;(3)磷酸丙糖異構(gòu)酶的表達在配對的正常肺組織和肺腺癌組織中有差異統(tǒng)計學(xué)意義,且P0.01;(4)磷酸丙糖異構(gòu)酶的表達在肺良性病變組織和肺腺癌組織中有差異統(tǒng)計學(xué)意義,P0.01。結(jié)論:(1)術(shù)前的肺良性病變組與肺腺癌組的血清TPI濃度值不具有差異性;(2)肺良性病變組中的術(shù)前與術(shù)后的血清TPI濃度值無明顯變化;(3)肺腺癌組中術(shù)后較術(shù)前的血清TPI濃度值有所下降,但無統(tǒng)計學(xué)意義;(4)手術(shù)治療方式對術(shù)前術(shù)后的血清TPI濃度的變化在肺良性病變組與肺腺癌組中無明顯差異。(5)肺腺癌癌旁正常肺組織與肺部良性病變組織中磷酸丙糖異構(gòu)酶的表達無明顯統(tǒng)計學(xué)差異,即磷酸丙糖異構(gòu)酶在腺癌中高表達,在正常肺組織及肺部良性病變組織中不表達或弱陽性表達,提示磷酸丙糖異構(gòu)酶是肺腺癌的差異表達的蛋白質(zhì)。(6)因為磷酸丙糖異構(gòu)酶在肺腺癌患者與肺良性病變患者的血清中無差異表達,故此酶用于肺腺癌的血液檢測臨床檢驗意義不大。
[Abstract]:Objective: To study the triose phosphate isomerase (triose phosphate isomerase TPI) expression in human lung cancer tissues and adjacent normal lung tissues, and the differences in serum of preoperative and postoperative serum samples in the expression, and to explore the extent of expression of the enzyme in the serum and tissues had no correlation that is, whether can check instead of tissues detected by serum, and thus a more convenient and effective for the diagnosis and prognosis of lung adenocarcinoma was evaluated. Methods: (1) 44 cases of fresh human lung adenocarcinoma tissues and adjacent normal lung tissue from the tumor hospital of Hunan Province, without a chest radiotherapy, chemotherapy of lung cancer resection surgery lobectomy were diagnosed by pathology and Tumor Hospital of Hunan province for lung cancer (excluding other cancer tissue into points); (2) from 24 patients with benign lung disease group were from Thoracic Tumor Hospital of Hunan Province A surgical resection of the lung tissue, including pneumonia, inflammatory pseudotumor, pulmonary tuberculosis, all benign lung disease patients had no history of cancer; (3) from the 44 cases of patients with lung adenocarcinoma and 24 cases of blood samples and seventh days 10ml days before operation in 1 patients with benign lung diseases after operation the blood samples of 10ml. TPI were statistically analyzed the overall mean and statistical analysis, serum ELISA method using Wilcoxon rank sum test, the results of immunohistochemistry and Wilcoxon test. Results: the results of ELISA analysis: (1) preoperative benign lung disease group (A1) and postoperative pulmonary benign lesions group (A2) compared and using the Wilcoxon test, Z=0.517 test, P0.05, bilateral P=0.605, no statistical significance; (2) preoperative pulmonary adenocarcinoma group (B1) and postoperative pulmonary adenocarcinoma group (B2) compared with Wilcoxon rank test, Z=1.228 test, P0.05, bilateral P=0.219, no statistical significance (3; benign lung disease group) and lung adenocarcinoma Decline in cancer group compared with the values, using the Wilcoxon test, Mann-Whitney U=430; Wilcoxon W=706; Z=0.270, P=0.787 P0.05, two-sided test, no statistical significance; (4) the two groups compared to that of A1. Compared with B1, using the Wilcoxon test, Z=0.274 test, double side P=0.784, P0.05, no statistical significance. Immunohistochemical analysis of the statistical results: (1) the triose phosphate isomerase in benign tissues, lung adenocarcinoma tissues and matched normal lung tissue expression had significant differences, P0.01; (2) the triose phosphate isomerase was not statistically significant between the paired normal lung tissue and lung benign lesion tissues P0.05; (3) the expression of triosephosphate isomerase have statistically significant difference, in normal lung tissue and paired lung cancer tissues and P0.01; (4) the expression of triosephosphate isomerase has statistically significant difference in benign lung lesions and lung cancer tissues Conclusion: P0.01. (1), benign lung disease group and lung cancer group, the serum TPI concentration of preoperative value does not have differences; (2) benign lung disease group in the preoperative and postoperative serum TPI concentration was not significantly different; (3) lung cancer group serum TPI concentration of after compared with the preoperative values have declined, but without statistical significance; (4) surgical treatment changes of serum concentration of TPI before and after surgery had no obvious difference in benign lung disease group and lung cancer group. (5) the expression of triosephosphate isomerase in lung adenocarcinoma and adjacent normal lung tissue pulmonary benign lesions was no significant difference, i.e. triosephosphate isomerase in adenocarcinoma with high expression, no expression or weak expression in normal lung tissue and lung benign lesion tissues, suggesting that triose phosphate isomerase is differentially expressed in lung adenocarcinoma protein. (6) because of the triose phosphate isomerase in patients with lung adenocarcinoma and lung benign There is no differential expression in the serum of the patients with sexual lesions, so the clinical test of the enzyme used in the detection of lung adenocarcinoma is of little significance.
