【摘要】：目的:觀(guān)察以帶蒂的隱神經(jīng)營(yíng)養(yǎng)皮瓣配合中藥治療一期修復(fù)踝關(guān)節(jié)損傷伴軟組織缺損的臨床療效。方法:對(duì)16例伴有膝踝關(guān)節(jié)部位軟組織缺損的踝關(guān)節(jié)損傷患者,先行急診清創(chuàng),再設(shè)計(jì)以帶蒂隱神經(jīng)營(yíng)養(yǎng)皮瓣,對(duì)缺損的軟組織進(jìn)行修復(fù),術(shù)后內(nèi)服中藥湯劑治療。結(jié)果:本組16例所有皮瓣術(shù)后充盈良好;4例出現(xiàn)瘀血情況,經(jīng)處理后好轉(zhuǎn);1例帶血管蒂的島狀皮瓣邊緣出現(xiàn)壞死,經(jīng)換藥后愈合。供區(qū)創(chuàng)面均一期愈合。隨訪(fǎng)10～24月,平均16月,結(jié)果1例作二期修整外,其余皮瓣外形滿(mǎn)意�；减�、足按美國(guó)足踝外科協(xié)會(huì)踝部評(píng)分標(biāo)準(zhǔn)進(jìn)行評(píng)估,結(jié)果優(yōu)6例,良9例,可1例,差0例,優(yōu)良率為93.75%。結(jié)論:帶蒂的隱神經(jīng)營(yíng)養(yǎng)皮瓣配合中藥內(nèi)服是治療伴有軟組織缺損的踝關(guān)節(jié)損傷的有效方法。
[Abstract]:Objective: To observe the clinical efficacy of pedicled saphenous neurotrophic flap combined with traditional Chinese medicine in the treatment of one stage repair of ankle joint injury with soft tissue defect. Methods: 16 patients with ankle joint injury with soft tissue defect of the knee and ankle joint were treated in the emergency debridement first, and the skin flap was redesigned with pedicled hidden God, and the soft tissue of the defect was repaired. Results: 16 cases of all skin flaps in this group were filled with good filling, 4 cases had blood stasis and improved after treatment; 1 cases of island flap with vascular pedicle appeared necrotic and healed after changing the medicine. The follow-up was 10~24 months, the average of 16 months was 16 months, the results of the 1 cases were full and the remaining flaps were full shape. According to the ankle score standard of the American ankle surgery association, the results were excellent in 6 cases, good in 9 cases, in 1 cases and poor in 0 cases. The good rate was 93.75%. conclusion: the pedicled saphenous neurocutaneous flap combined with Chinese medicine was an effective method for the treatment of ankle joint injury with soft tissue defect.
