【摘要】：目的:探討多囊卵巢綜合征(PCOS)患者早期自然流產(chǎn)與子宮內(nèi)膜雌激素受體(ER)、孕激素受體(PR)的關(guān)系以及中西醫(yī)結(jié)合治療的可行性。方法:選擇PCOS不孕患者62例為觀察組,同期輸卵管性不孕患者30例為對(duì)照組。兩組患者均于月經(jīng)干凈3～7d時(shí)在宮腔鏡下取子宮內(nèi)膜標(biāo)本,行免疫組織化學(xué)檢查,觀察ER和PR在子宮內(nèi)膜腺體和間質(zhì)細(xì)胞的表達(dá)強(qiáng)度,比較兩組的差異。再于術(shù)后對(duì)觀察組對(duì)象給予達(dá)英-35聯(lián)合中藥天癸方連續(xù)治療3個(gè)月經(jīng)周期,停藥后第1次月經(jīng)干凈3~7d在宮腔鏡引導(dǎo)下取子宮內(nèi)膜標(biāo)本,觀察治療前后ER、PR表達(dá)強(qiáng)度的變化。結(jié)果:ER、PR在子宮內(nèi)膜腺體和間質(zhì)中的表達(dá)觀察組較對(duì)照組明顯減弱(P0.05);經(jīng)中西醫(yī)結(jié)合治療3個(gè)周期后,觀察組患者子宮內(nèi)膜腺體和間質(zhì)中的ER、PR表達(dá)強(qiáng)度均明顯提高(P0.05)。結(jié)論:PCOS患者子宮內(nèi)膜ER、PR減少可能是其自然流產(chǎn)的重要原因,中西醫(yī)結(jié)合治療能有效提高PCOS患者子宮內(nèi)膜ER、PR水平。
[Abstract]:Objective: to investigate the relationship between early spontaneous abortion and endometrial estrogen receptor (ER) and progesterone receptor (PRP) in patients with polycystic ovary syndrome (PCOS). Methods: 62 cases of PCOS infertility were selected as observation group and 30 cases of oviduct infertility as control group. The samples of endometrium were collected under hysteroscopy and the expression of ER and PR in endometrial glands and stromal cells were observed by immunohistochemistry. The difference between the two groups was compared. After operation, the patients in the observation group were treated with Daxin-35 combined with Tiangui recipe for 3 consecutive menstrual cycles. The endometrial specimens were collected under hysteroscopic guidance for 3 days after the first menstruation and the changes of ERPR expression before and after treatment were observed. Results the expression of ERPR in endometrial glands and stroma in the observation group was significantly lower than that in the control group, and the expression of ERPR in the endometrial glands and stroma of the observation group was significantly increased after three cycles of treatment with the combination of traditional Chinese and western medicine (TCM-WM), and the expression intensity of ERPR in the endometrial glands and stroma of the observation group was significantly higher than that of the control group. Conclusion the decrease of endometrial ERPR may be an important cause of spontaneous abortion in patients with PCOS, and the combination of traditional Chinese medicine and western medicine can effectively improve the level of ERPR in endometrium of PCOS patients.
