文章摘要:目的研究針刺腧穴引起的中樞效應(yīng).探討不同敏感度體質(zhì)因素對(duì)針刺鎮(zhèn)痛的影響。方法篩選3組典型體質(zhì)的健康志愿者,分為不敏感組、正常組和敏感組,每組15例。測(cè)定各組針刺左足三里穴前后的試驗(yàn)壓痛閾值,應(yīng)用配對(duì)t檢驗(yàn)比較鎮(zhèn)痛效應(yīng)。2周后,應(yīng)用功能磁共振成像(fMRI)技術(shù)對(duì)受試者針刺前后進(jìn)行靜息態(tài)血氧水平依賴(bloodoxygen level dependent,BOLD)掃描,計(jì)算并比較各組低頻振幅(amplitude of low frequencyfluctuation,ALFF)值。結(jié)果①正常組和敏感組針刺前后壓痛閾值差異均有統(tǒng)計(jì)學(xué)意義(P〈0.05)。②與針刺前比較,針刺后不敏感組在左顳下回、左梭狀回ALFF值降低,,未見ALFF值增高腦區(qū);正常組在雙側(cè)小腦后葉、腦干、邊緣葉、顳下回、右尾狀核ALFF值增高;在左中央后回、左中央前回、雙側(cè)枕葉ALFF值減低;敏感組在雙側(cè)小腦后葉、顳下回、梭狀回、額葉、基底節(jié)、前扣帶回、右緣上回、右補(bǔ)充運(yùn)動(dòng)區(qū)、左島葉ALFF值增高;在左顳中回、左中央后回、后扣帶回、雙側(cè)枕葉ALFF值減低。結(jié)論不同敏感度體質(zhì)因素影響針刺鎮(zhèn)痛效應(yīng),其中樞神經(jīng)調(diào)控機(jī)制可能與針刺引起不同的腦區(qū)響應(yīng)網(wǎng)絡(luò)相關(guān)。
Abstr:Objective To determine the analgesia effect in different sensitive constitution and investi- gate its underlying central nerve modulating mechanism. Methods Forty-five healthy volunteers were recruited and divided into 3 typical groups according to Traditional Chinese Medicine theory on constitution, that is, insensitive group, normal group and sensitive group, with 15 cases in each group. Pressure pain threshold (PPT) was measured in every group before and after acupuncture at Zusanli point (ST36). Then paired t test was used to compare the effect of analgesia among the 3 groups. Two weeks later, resting-state BOLD fMRI scanning was carried out by using a gradient-echo echo-planar imaging sequence before and after acupuncture at ST36. All the {MRI data were analyzed by the method of amplitude of low frequency fluctuation (ALFF). Paired t-test was used to analyze the data acquired before and after acupuncture. Results Compared the PPT before and after acupuncture, normal group and sensitive group had obvious statistical significance (P 〈 0. 05 ). Compared with the image before acupuncture, the insensitive group showed significant decreased ALFF values in the left fusiform gyrus and left inferior temporal gyrus. No increased ALFF was found in this group. The normal group had significantly increased ALFF values in the bilateral cerebellum posterior lobe, brainstem, limbic lobe, left inferior temporal gyms, and right caudate, and significantly decreased ALFF values in the left postcentralgyrus , left precentral gyrus, and bilateral occipital lobes. The ALFF values in the sensitive group were increased mainly in the bilateral cerebellum posterior lobe, fusiform gyms, inferior temporal gyrus, basal ganglia, frontal lo
