【摘要】：目的觀察經(jīng)皮微創(chuàng)針刀治療拇指狹窄性腱鞘炎的療效。方法采用局麻下針刀微創(chuàng)治療70例拇指狹窄性腱鞘炎患者,均為單側(cè)發(fā)病,共70個患指。術中對A1滑車部進行切割松解,術后自針刀向腱鞘部注入復方倍他米松。術前術后參照Cebesoy方法行拇指5種特定功能活動時VAS評分,評定療效,同時觀察并發(fā)癥。結(jié)果手術時間平均(8.5±1.31)min。拇指狹窄性腱鞘炎一次針刀治療后完全緩解63例,松解欠徹底7例,2周后行第2次針刀術,2次術后癥狀緩解。術后1個月VAS優(yōu)良率94.3%,術后6個月VAS優(yōu)良率100.0%。術前拇指5種特定功能活動下的VAS總評分為29.37±5.29,術后1個月降低為3.12±1.96,術后6個月為2.44±1.52。術后1個月與6個月比較,VAS評分差異有統(tǒng)計學意義。隨訪未見重大不良反應。結(jié)論經(jīng)皮微創(chuàng)針刀治療拇指狹窄性腱鞘炎能夠有效緩解患指疼痛癥狀,快速恢復患指活動功能,安全可靠。
[Abstract]:Objective to observe the therapeutic effect of percutaneous minimally invasive needle knife on tenosynovitis of thumb. Methods 70 cases of thumb stenosis tenosynovitis were treated with needle knife under local anesthesia. The A 1 trochlear region was cut and loosened during the operation. After operation, compound betamethasone was injected into the tendon sheath from the needle knife. According to the Cebesoy method, the VAS score of five specific functional activities of thumb was evaluated before and after operation, the curative effect was evaluated, and complications were observed at the same time. Results the average operation time was 8.5 鹵1.31 min. 63 cases of tenosynovitis tenosynovitis of thumb were completely relieved after one acupuncture operation, and 7 cases were relieved completely 2 weeks later, the symptoms were relieved 2 times after the second acupuncture operation. The excellent and good rate of VAS was 94. 3% at 1 month after operation, and the excellent and good rate of VAS was 100. 0 at 6 months after operation. The total score of VAS was 29.37 鹵5.29, 3.12 鹵1.96 and 2.44 鹵1.52respectively. There was significant difference in VAS score between 1 month and 6 months after operation. No significant adverse reactions were observed during follow-up. Conclusion the percutaneous minimally invasive needle-knife treatment of thumb stenosis tenosynovitis can effectively relieve the pain symptoms of the affected fingers and restore the motility function of the affected fingers quickly, which is safe and reliable.
