【摘要】：目的評價中西醫(yī)聯(lián)合治療眼底出血的有效性和安全性。方法計算機檢索Pub Med、Cochrane Library、中國知網(wǎng)、維普網(wǎng)、中國生物醫(yī)學(xué)文獻數(shù)據(jù)庫、萬方數(shù)據(jù)知識服務(wù)平臺中收錄的有關(guān)中西醫(yī)聯(lián)合治療與西醫(yī)治療眼底出血的中、英文文獻,手工檢索相關(guān)的參考文獻,檢索時間為建庫至2017年4月。納入比較中西醫(yī)聯(lián)合治療與西醫(yī)治療眼底出血的隨機對照試驗。采用Cochrane系統(tǒng)評價手冊評估文獻質(zhì)量;采用Review Manager 5.3軟件對臨床療效進行Meta分析。結(jié)果共納入16篇文獻,共1 821只眼(中西醫(yī)聯(lián)合治療眼底出血組925只眼,西醫(yī)治療眼底出血組896只眼),均為隨機對照試驗。Meta分析結(jié)果顯示,中西醫(yī)聯(lián)合治療眼底出血組的臨床療效好于西醫(yī)治療眼底出血組,差異有統(tǒng)計學(xué)意義[WMD=3.58,Z=9.38,95%CI(2.74,4.67),P0.001]。臨床療效倒漏斗圖相對對稱,提示發(fā)表偏倚較小。結(jié)論中西醫(yī)聯(lián)合治療眼底出血的臨床療效優(yōu)于西醫(yī)治療眼底出血。所納入的部分文獻中未記錄隨機分組的方法和具體藥物使用情況,所以文獻的總體質(zhì)量不高,仍需大量高質(zhì)量的隨機對照試驗來證實中西醫(yī)聯(lián)合治療眼底出血的有效性。但中西醫(yī)聯(lián)合治療給眼底出血的患者帶來了新希望。在臨床實踐中將中醫(yī)的辨證論治與西醫(yī)治療眼底出血更完美的結(jié)合,給患者的預(yù)后帶來更好的長期療效。
[Abstract]:Objective to evaluate the efficacy and safety of combined traditional Chinese and western medicine in the treatment of fundus hemorrhage. Methods the Chinese and English literatures on the treatment of ocular fundus hemorrhage were searched by computer in Pub Medsite Cochrane Library, China knowledge Network, Weip Web, Chinese Biomedical Literature Database and Wanfang data knowledge Service platform. Manual retrieval of relevant references, retrieval time for the establishment of the database until April 2017. A randomized controlled trial was conducted to compare the combined treatment of traditional Chinese and western medicine with that of western medicine in the treatment of ocular fundus hemorrhage. The literature quality was evaluated by Cochrane system evaluation manual and Meta analysis was performed by Review Manager 5.3 software. Results A total of 1 821 eyes (925 eyes in the fundus hemorrhage group treated with traditional Chinese and western medicine and 896 eyes in the fundus hemorrhage group treated with western medicine) were included in 16 articles. The clinical effect of combined TCM and Western medicine in treating fundus hemorrhage group was better than that in western medicine group, and the difference was statistically significant. The clinical efficacy of inverted funnel graph is relatively symmetrical, suggesting that the publication bias is small. Conclusion the therapeutic effect of combined traditional Chinese and western medicine on fundus hemorrhage is better than that on western medicine. The method of random grouping and the use of specific drugs were not recorded in part of the literature, so the overall quality of the literature is not high, and a large number of high quality randomized controlled trials are still needed to confirm the effectiveness of combined TCM and Western medicine in the treatment of fundus hemorrhage. But the combined treatment of traditional Chinese and western medicine brings new hope to the patients with fundus hemorrhage. In clinical practice, the combination of TCM syndrome differentiation and western medicine in the treatment of ocular fundus hemorrhage is more perfect, which will bring better long-term curative effect to the prognosis of patients.
