【摘要】：目的 利用疼痛評分指標觀察并對比鹽酸曲馬多注射液穴位注射與肌肉注射在治療癌痛患者出現(xiàn)爆發(fā)性疼痛的臨床療效,為爆發(fā)性疼痛患者迅速止痛尋求更有效的具有中醫(yī)特色的治療方法。 方法 采用隨機、對照的試驗研究方法,將符合納入標準的60例爆發(fā)性疼痛病例按簡單隨機法分為治療組與對照組,其中治療組29例,對照組31例,分別予鹽酸曲馬多注射液足三里穴位注射和肌肉注射,觀察并記錄疼痛開始緩解時間、比較不同時間段數(shù)字分級法(numerical rating scale,NRS)疼痛評分以及不良反應,初步探討足三里穴位注射止痛藥物在治療爆發(fā)性疼痛患者的臨床療效。 結果 經(jīng)治療后,可看到治療組的起效時間明顯短于對照組。治療后治療組及對照組的NRS評分均呈下降趨勢,但在治療后0.5小時兩組數(shù)值接近,差異無統(tǒng)計學意義(P0.05),在1小時開始兩組的NRS評分差異有統(tǒng)計學意義(P0.05),提示隨著時間的推移,治療組的下降趨勢與對照組不一致且優(yōu)于對照組,至治療后5小時兩組的NRS評分均有反彈,但治療組的NRS評分始終低于對照組,提示足三里穴位注射在維持療效方面優(yōu)于肌肉注射。 通過觀察,兩組患者在觀察期間出現(xiàn)不良反應的對比差異無統(tǒng)計學意義。 結論 足三里穴位注射鹽酸曲馬多注射液相對比肌肉注射在治療爆發(fā)性疼痛具有起效時間更短,更有效緩解疼痛,而沒有增加鹽酸曲馬多注射液的不良反應,是簡單易行、安全有效的治療爆發(fā)性癌痛的手段。
[Abstract]:Purpose. The clinical efficacy of tramadol hydrochloride injection at acupoint and intramuscular injection in the treatment of explosive pain in patients with cancer pain was observed and compared with the pain score index. To seek a more effective treatment with TCM characteristics for patients with explosive pain. Method. A randomized, controlled trial was conducted to divide 60 patients with burst pain into treatment group (n = 29) and control group (n = 31) according to the simple randomized method, including 29 cases in treatment group, 31 cases in control group, 31 cases in control group, 29 cases in treatment group and 31 cases in control group. Tramadol hydrochloride injection was given to Zusanli point injection and intramuscular injection to observe and record the onset of pain relief. The pain scores and adverse reactions of numerical rating scale-NRSs in different time periods were compared. To explore the clinical effect of Zusanli acupoint injection analgesic drug in the treatment of explosive pain patients. Results. After treatment, the effective time of the treatment group was significantly shorter than that of the control group. The NRS scores of the treatment group and the control group showed a downward trend after treatment, but the values of the two groups were similar at 0.5 hours after treatment. There was no significant difference in NRS scores between the two groups at 1 hour. It was suggested that the decreasing trend of the treatment group was different from that of the control group and was superior to that of the control group with the passage of time. Up to 5 hours after treatment, the NRS score of both groups rebounded, but the NRS score of the treatment group was always lower than that of the control group, indicating that Zusanli acupoint injection was superior to intramuscular injection in maintaining the curative effect. There was no significant difference in adverse reactions between the two groups. Conclusion. Tramadol hydrochloride injection at Zusanli acupoint has a shorter onset time and more effective pain relief than muscle injection, but it is simple and easy to do without increasing the adverse reactions of tramadol hydrochloride injection. A safe and effective method for the treatment of explosive cancer pain.
