【摘要】：目的觀察中西醫(yī)結(jié)合治療頑固性鼻出血的臨床療效。方法 90例頑固性鼻出血患者隨機(jī)分為鼻內(nèi)鏡組、阿莫西林組、一清膠囊組,每組30例。鼻內(nèi)鏡組使用鼻內(nèi)鏡微波治療;阿莫西林組在鼻內(nèi)鏡微波治療后服用阿莫西林膠囊,每次0.5g,每天3次,連服3~5天;一清膠囊組在阿莫西林組治療基礎(chǔ)上再服用一清膠囊,每次1g,每日3次,連服2周。對患者治療前后尿激酶型纖溶酶原激活物(uPA)及其受體(uPAR)、凝血酶原時間(PT)、活化部分凝血活酶時間(APTT)、凝血酶時間(TT)及血漿纖維蛋白原水平(FIB)進(jìn)行檢測。同時觀察臨床療效、治愈次數(shù)。結(jié)果一清膠囊組臨床療效總有效率90.0%,顯著高于鼻內(nèi)鏡組的63.3%(P0.05),與阿莫西林組的80.0%比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。一清膠囊組一次治愈率顯著高于鼻內(nèi)鏡組與阿莫西林組(P0.05)。治療前后鼻內(nèi)鏡組uPA、PT、FIB,阿莫西林組uPA、PT、APTT、FIB,一清膠囊組uPA、uPAR、PT、APTT、TT及FIB比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05);治療后一清膠囊組uPA、uPAR、APTT、TT、FIB與鼻內(nèi)鏡組及阿莫西林組,PT與鼻內(nèi)鏡組比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論中西醫(yī)結(jié)合治療頑固性鼻出血可提高臨床療效,改善凝血指標(biāo)。
[Abstract]:Objective to observe the clinical effect of combined traditional Chinese and western medicine on intractable epistaxis. Methods 90 patients with intractable epistaxis were randomly divided into three groups: nasal endoscope group, amoxicillin group and Yiqing capsule group. There were 30 cases in each group. Amoxicillin group was treated with amoxicillin capsule (0.5 g, 3 times a day, 3 days) after endoscopic microwave therapy. The Yiqing capsule group was treated with Yiqing capsule on the basis of amoxicillin treatment, 1 g each time, 3 times a day. Before and after treatment, urokinase type plasminogen activator (UPA) and its receptor, prothrombin time (PTT), activated partial thromboplasminogen activator (APTT). Thrombin time (TTT) and plasma fibrinogen level were measured. The clinical efficacy and cure times were observed. Results the total effective rate of Yiqing capsule group was 90.0%. It was significantly higher than that in nasal endoscopy group (63.3%) and amoxicillin group (80.0%%). The cure rate of Yiqing capsule group was significantly higher than that of nasal endoscopy group and amoxicillin group (P 0.05). In amoxicillin group, there were significant differences in APTTT TT and FIB between Amoxicillin group and Yiqing capsule group (P 0.05). After treatment, the patients in the Yiqing capsule group were treated with TTFIB, UPA-UPA-UPARP-APTTTI-TTFIB, nasal endoscopy group and amoxicillin group. Conclusion the treatment of intractable epistaxis with combination of traditional Chinese medicine and western medicine can improve the clinical efficacy and blood coagulation index.
