軍隊(duì)老年男性代謝綜合征與血清IL-10、hs-CRP的關(guān)系

【摘要】: 目的觀察軍隊(duì)老年男性代謝綜合征(metabolic syndrome,MS)患者血清白細(xì)胞介素10(Interleukin-10,IL-10)、高敏C反應(yīng)蛋白(high sensitive C reactive protein,hs-CRP)水平的變化,探討炎癥標(biāo)志物IL-10、hs-CRP與代謝綜合征的關(guān)系。 方法對(duì)2006年11月至12月間在163醫(yī)院門(mén)診體檢的408名軍隊(duì)退休男性老干部測(cè)定腰圍(waist circumference,WC)、體重、身高、血壓、空腹血糖(fasting plasma glucose,F(xiàn)PG)、血脂水平,并計(jì)算體重指數(shù)(body mass Index,BMI),分析其代謝紊亂的特征。據(jù)國(guó)際糖尿病聯(lián)盟(IDF)頒布的MS診斷標(biāo)準(zhǔn),入選58例老年MS患者和29例年齡相匹配的對(duì)照者,測(cè)量其血壓、腰圍(WC)、體重、身高、體重指數(shù)(BMI)、血脂、空腹血糖(FPG)、空腹血胰島素(fastinginsulin,F(xiàn)INS)、抵抗指數(shù)(homeostasis model assessment of Insulin Resistance,HOMA-IR),血清IL-10濃度用酶聯(lián)免疫吸附劑法測(cè)定(enzyme linked immunosorbent assay,,ELISA),血清hs-CRP水平用乳膠增強(qiáng)免疫比濁法檢測(cè)。 結(jié)果1.408名軍隊(duì)退休男性老干部中超重或肥胖者237例(占58.1%),中心性肥胖者(腰圍≥90cm)243例(占51.6%),高血壓病者287例(占70.3%),高血糖患者135例(占33.1%),血脂紊亂者(高TG或/和低HDL-C)95例(占23-3%),MS患者135例(占33.1%);可見(jiàn)2種代謝紊亂組中以高血壓+超重或肥胖組合(占58.9%)最多,3種代謝紊亂組中以高血壓+超重或肥胖+血脂紊亂組合(占57.5%)最多;有3種以上MS相關(guān)疾病者占該人群總數(shù)的近30%。2.MS組血清IL-10濃度顯著低于對(duì)照組(P<0.05);按HOMA-IR分組發(fā)現(xiàn):胰島素抵抗亞組(HOMA-IR≥4.3 1)IL-10水平顯著低于非胰島素抵抗亞組(HOMA-IR<4.3 1)(P=0.000)。3.MS患者血清hs-CRP濃度顯著高于對(duì)照者(P<0.05);按HOMA-IR、WC分組發(fā)現(xiàn):非胰島素抵抗亞組(HOMA-IR<4.31)hs-CRP水平低于胰島素抵抗亞組(HOMA-IR≥4.31) (P>0.05);中心性肥胖亞組(WC≥90cm)血清hs-CRP濃度顯著高于非中心性肥胖亞組(WC<90cm) (P=0.008);MS患者血清hs-CRP水平隨著MS組分?jǐn)?shù)目的增加而升高。4.Pearson相關(guān)分析顯示在MS組中,IL-10與HOMA-IR(r=-0.797,P=0.000)、FINS(r=-0.731,P=0.000)、FPG(r=-0.303,P=0.021)之間存在負(fù)相關(guān)。在MS組中,hs-CRP與年齡(r=0.270,P=0.040)、BMI(r=0.346,P=0.008)之間存在正相關(guān)。5.Stepwise逐步回歸分析顯示在MS組中,腰圍、HOMA—IR、FINS可能為IL-10的影響因素;在MS組中,年齡、BMI可能為hs-CRP的影響因素; 結(jié)論1.408名軍隊(duì)退休男性老干部中MS相關(guān)疾病患者所占比例較高。2.老年男性MS患者血清IL-10濃度顯著降低血清hs-CRP顯著升高,提示存在低度慢性炎癥。3.老年男性MS,患者血清IL-10濃度降低,可能與高胰島素血癥、胰島素抵抗密切相關(guān)。4.老年男性MS患者血清hs-CRP水平隨著MS組分?jǐn)?shù)目的增加而升高,并可能與年齡、BMI相關(guān)。
