【摘要】：目的探討中美兩軍空軍飛行學(xué)員選拔膝內(nèi)、外翻醫(yī)學(xué)標(biāo)準(zhǔn)的不同,并對我軍招飛體檢標(biāo)準(zhǔn)改革提供思路。方法調(diào)查我軍2012~2015年招飛定選體檢中膝內(nèi)、外翻淘汰率與綜合評定率,對比我空軍與美空軍飛行學(xué)員膝內(nèi)、外翻醫(yī)學(xué)選拔標(biāo)準(zhǔn),并進(jìn)行實(shí)證研究。結(jié)果每年因膝內(nèi)、外翻淘汰人數(shù)均較少,4年共18人,占骨科疾病淘汰人數(shù)的9.1%;膝內(nèi)、外翻應(yīng)征者中2014年獲得綜合評定合格資格4人,2015年為31人,占骨科疾病獲得綜評合格總?cè)藬?shù)的15%。我軍對膝內(nèi)、外翻的篩選淘汰標(biāo)準(zhǔn)為形態(tài)學(xué)標(biāo)準(zhǔn),美軍標(biāo)準(zhǔn)沒有直接的膝內(nèi)、外翻標(biāo)準(zhǔn)條目,而是從功能出發(fā),對膝內(nèi)、外翻可能造成的癥狀和體征進(jìn)行了全面的規(guī)定。我軍2012~2015年因膝內(nèi)、外翻而淘汰的18名應(yīng)征者中,按照美軍標(biāo)準(zhǔn)應(yīng)有9人為合格。2014~2015年綜評合格的35名膝內(nèi)、外翻應(yīng)征者中,按照美軍標(biāo)準(zhǔn)也應(yīng)有32名合格。結(jié)論我軍目前膝內(nèi)、外翻標(biāo)準(zhǔn)準(zhǔn)確性較低,建議參考美軍標(biāo)準(zhǔn)側(cè)重功能評估,同時引入脛股角概念,改進(jìn)形態(tài)學(xué)標(biāo)準(zhǔn)。
[Abstract]:Objective to explore the difference of the selection standard of Chinese and American air force cadets and to provide some ideas for the reform of the standard of medical examination. Methods to investigate the selection rate and comprehensive evaluation rate of invagination and valgus in our army during the period of 2012-2015, and to compare the selection standard of Chinese Air Force and American Air Force flight cadets in knee and valgus, and to carry out an empirical study. Results each year, the number of valgus elimination was relatively small, accounting for 9. 1% of the total number of orthopaedic diseases eliminated in 4 years, and 4 of the patients who applied for valgus were qualified for comprehensive evaluation in 2014 and 31 in 2015. It accounted for 15% of the total number of patients with orthopaedic diseases who were qualified for comprehensive assessment. The criteria for selection and elimination of knee and valgus in our army are morphologic criteria. The US military standard does not have direct items for knee and valgus, but from the point of view of function, it makes a comprehensive regulation on the symptoms and signs that may be caused by varus in the knee. Of the 18 candidates eliminated in our army from 2012 to 2015 due to knee valgus, according to the US military standard, 9 should be qualified. In 2014-2015, 35 qualified candidates should be qualified, and among the valgus candidates, 32 should be qualified according to the US military standard. Conclusion at present, the accuracy of the varus criterion of our army is low. It is suggested to refer to the US military standard to emphasize the function evaluation, to introduce the concept of tibiofemoral angle and to improve the morphologic standard at the same time.
