模擬野戰(zhàn)條件下環(huán)境下應急采血的血液質(zhì)量評價

【摘要】:目的戰(zhàn)時應急采血是緊急狀況下傷員輸血救治的一項重要措施。文中模擬野戰(zhàn)高溫高濕環(huán)境下應急采血,從細菌污染和紅細胞質(zhì)量兩方面探討和評價戰(zhàn)時應急采血的血液質(zhì)量。方法在獻血屋內(nèi)對70名獻血員常規(guī)安爾碘消毒后采血70份作對照組,另在模擬野外環(huán)境下對70名獻血員先從內(nèi)至外全手臂內(nèi)側(cè)下2/3安爾碘擦拭2次后再常規(guī)2次安爾碘擦拭消毒后采血70份作為試驗組。其中每組各50份用于制備濃縮血小板,并進行細菌培養(yǎng);20份對照組血液采集后置2~6℃冰箱存放,1~2 h后分離出紅細胞懸液;20份試驗組血液長途海陸運輸4 h后分離出紅細胞懸液。保存期內(nèi)分別檢測紅細胞質(zhì)量參數(shù)(紅細胞ATP、細胞外液FHb、細胞外液K+、棘形紅細胞率)。結果 2組共100份濃縮血小板細菌培養(yǎng),均未檢出細菌。隨著儲存時間的延長,2組紅細胞ATP含量均下降,差異無統(tǒng)計學意義(P0.05)。2組紅細胞細胞外液FHb濃度隨貯存時間的延長而逐漸升高,試驗組第34天FHb濃度[(1356.1±813.6)mg/L]明顯高于對照組[(652.9±423.0)mg/L],差異有統(tǒng)計學意義(P0.01)。2組棘形紅細胞率隨貯存時間的延長而逐漸升高,試驗組第34天棘形紅細胞率[(49.4±9.9)%]明顯高于對照組[(23.7±11.5)%],差異有統(tǒng)計學意義(P0.01)。采血第1天試驗組細胞外液K+濃度[(7.1±0.7)mmol/L]明顯高于對照組[(4.3±1.6)mmol/L],差異有統(tǒng)計學意義(P0.05),但隨著時間變化,2組K+濃度逐漸增高,變化趨勢基本一致,差異無統(tǒng)計學意義(P0.05)。結論模擬野外條件下采血,通過增加消毒次數(shù)改善衛(wèi)生條件,血液質(zhì)量并未產(chǎn)生嚴重的質(zhì)量問題,但受高溫,山路、水路顛簸及長途運輸?shù)木C合影響,血液應在血液貯存期(35 d)內(nèi)盡早使用。
[Abstract]:Objective the emergency blood collection is an important measure for the blood transfusion of the wounded in wartime. This paper simulates the emergency blood collection under high temperature and high humidity in the field, and discusses and evaluates the blood quality of emergency blood collection in wartime from the aspects of bacterial contamination and red blood cell quality. Methods 70 blood donors were collected in the blood donation room after disinfection of routine Anor iodine as the control group.
