火器—凍復(fù)合傷后早期血液心肌酶組合變化研究

【摘要】:目的:基于寒冷地區(qū)發(fā)生局部戰(zhàn)爭或恐怖襲擊或其他突發(fā)事件的客觀事實(shí),瞄準(zhǔn)寒冷地區(qū)防御作戰(zhàn)、執(zhí)行非戰(zhàn)爭任務(wù),建立火器-凍、凍-火器復(fù)合傷動(dòng)物模型,揭示寒冷地區(qū)火器-凍、凍-火器復(fù)合傷后早期血液心肌酶組合改變特點(diǎn)和規(guī)律。為提高寒冷地區(qū)火器-凍、凍-火器復(fù)合傷救護(hù)水平與制定具體措施,減少因血液心肌酶組合改變所帶來的傷亡奠定基礎(chǔ)。 方法:將30只普通級日本大耳白兔隨機(jī)分為五組:A火器-凍傷組(n=6),B凍-火器傷組(n=6),C火器傷組(n=6),D凍傷組(n=6),E未受傷對照組(n=6)。各組制備相應(yīng)的動(dòng)物模型;(75±50)min后經(jīng)頸動(dòng)脈測壓管采血測肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、乳酸脫氫酶(LDH)、天門冬氨酸氨基轉(zhuǎn)移酶(AST)、丙氨酸氨基轉(zhuǎn)移酶(ALT)、α-羥丁酸脫氫酶(α-HBDH),分析各組受傷后早期血液心肌酶組合變化。 結(jié)果:各組動(dòng)物模型構(gòu)建成功。A組與對照組E相比:CK、CK-MB、LDH、AST、ALT、α-HBDH升高有非常顯著性差異(P<0.01);B組與對照組E相比:CK、CK-MB、LDH、AST、ALT、α-HBDH升高有顯著性差異(P<0.05);C組與對照組E相比:CK、CK-MB、LDH、AST、α-HBDH升高有顯著性差異(P<0.05),ALT雖無顯著性差異(P<0.05),但ALT有上升趨勢;D組與對照組E相比:CK、CK-MB、LDH、AST、ALT、α-HBDH雖無顯著性差異(P<0.05),但都有上升趨勢。A組和B組相比:CK、CK-MB、LDH、AST、ALT、α-HBDH升高有顯著性差異(P<0.05)。A、B組與C組相比:CK、CK-MB、LDH、AST、ALT、α-HBDH升高有顯著性差異(P<0.05)。A、B組與D組相比:CK、CK-MB、LDH、AST、ALT、α-HBDH升高有顯著性差異(P<0.05)。C組與D組相比:CK、CK-MB、LDH、AST、α-HBDH升高有顯著性差異(P<0.05),ALT雖無顯著性差異(P>0.05),但ALT有上升趨勢。 結(jié)論:火器-凍、凍-火器復(fù)合傷不僅有火器傷因素作用,還有低溫環(huán)境因素作用,兩種因素同時(shí)作用。與單一傷相比,火器-凍復(fù)合傷后血清心肌酶升高最明顯,組織損傷最為嚴(yán)重;凍-火器復(fù)合傷后血清心肌酶升高明顯,提示組織損傷較為嚴(yán)重;火器傷后血清心肌酶升高,提示組織損傷嚴(yán)重;凍傷后血清心肌酶有升高趨勢,提示組織有輕微損傷。
[Abstract]:Objective: based on the objective facts of local war or terrorist attack or other unexpected events in cold areas, aiming at the defense operations in cold regions, carrying out non-war tasks, establishing the animal model of firearms-frozen, frost-firearm combined injury,To reveal the characteristics and regularity of the changes of myocardial enzyme combination in the early stage after firearm-frozen and frozen-firearm combined injury in cold area.In order to improve the rescue level of firearm-frozen, frozen-firearm combined injury and make specific measures to reduce the casualties caused by changes of blood myocardial enzyme combination.Methods: thirty Japanese white rabbits of general grade were randomly divided into five groups: 1. A firearm-frostbite group (n = 6) and a firearm injury group (n = 6), a firearm injury group (n = 6) and a firearm injury group (n = 6). The frostbite group (n = 6) was divided into 5 groups (n = 5) and the control group (n = 6) was uninjured.In each group, 75 鹵50)min blood samples were collected to measure creatine kinase (CK), creatine kinase isoenzyme (CK-MBN), lactate dehydrogenase (LDH), aspartate aminotransferase (AST), alanine aminotransferase (alt), 偽 -hydroxybutyrate dehydrogenase (偽 -HBDH).The changes of myocardial enzyme composition in the early period after injury were analyzed.Results: compared with the control group E, there were very significant differences in the elevation of ALT, 偽 -HBDH and 偽 -HBDH between the two groups (P < 0.01). There were significant differences in the elevation of ALT, 偽 -HBDH and 偽 -HBDH between the two groups (P < 0.05) and the control group E (P < 0.05).Although there was no significant difference (P < 0.05), there was an increasing trend of ALT in group D (P < 0.05). Although there was no significant difference in ALT between group D and group E, 偽 -HBDH had no significant difference (P < 0.05), but there was an increasing trend in group A and group B. there was significant difference in the elevation of 偽 -HBDH between group A and group B (P < 0.05). There was a significant difference between group A and group C (P < 0.05). The increase of 偽 -HBDH in group A was significantly higher than that in group C (P < 0.05).The difference was significant (P < 0.05). Compared with D group, there was a significant difference in the elevation of 偽 -HBDH and 偽 -HBDH between group A and group D. There was no significant difference in the elevation of 偽 -HBDH between group C and group D (P < 0.05), but there was a trend of increase in ALT in group C and group D (P < 0.05), but there was no significant difference in the elevation of 偽 -HBDH between group A and group D (P < 0.05), but there was a trend of increase in ALT in group C (P < 0.05) compared with that in group D (P < 0.05), but there was no significant difference between group A and group D (P > 0.05).Conclusion: firearm-frozen, frozen-firearm combined injury not only has the effect of firearm injury, but also has the effect of low temperature environment factor, two kinds of factors acting at the same time.Compared with single injury, the increase of serum myocardial enzyme was the most obvious and the tissue injury was the most serious after firearm combined injury, and the increase of serum myocardial enzyme after firearm injury was significant, which indicated that the tissue injury was more serious, and the level of serum myocardial enzyme was higher after firearm injury.The results indicated that the tissue injury was serious and the serum myocardial enzyme increased after frostbite, suggesting slight tissue injury.
