【摘要】：目的:研究脾臟炎性假瘤樣濾泡樹(shù)突細(xì)胞瘤的CT表現(xiàn)及病理學(xué)特征。方法:回顧性分析2例經(jīng)手術(shù)和病理證實(shí)為炎性假瘤樣濾泡樹(shù)突細(xì)胞瘤的臨床特點(diǎn)、CT表現(xiàn)及病理資料,并復(fù)習(xí)相關(guān)文獻(xiàn),總結(jié)炎性假瘤樣濾泡樹(shù)突細(xì)胞瘤的CT及病理學(xué)特征。結(jié)果:2例炎性假瘤樣濾泡樹(shù)突細(xì)胞瘤CT平掃表現(xiàn)為類(lèi)圓形低密度腫塊影,邊界清晰,密度均勻,增強(qiáng)掃描動(dòng)脈期示病灶邊緣強(qiáng)化,門(mén)脈期病灶呈漸進(jìn)性持續(xù)強(qiáng)化并向中央填充。病理:炎性假瘤樣濾泡樹(shù)突細(xì)胞瘤除了梭形細(xì)胞和炎細(xì)胞外,還有由少許多核巨細(xì)胞和組織細(xì)胞構(gòu)成的伴有中心壞死的肉芽腫樣改變;EBER原位雜交:EBER-ISH(+)。結(jié)論:脾臟炎性假瘤樣濾泡樹(shù)突細(xì)胞瘤CT動(dòng)態(tài)增強(qiáng)掃描具有漸進(jìn)性、持續(xù)性強(qiáng)化的特點(diǎn),其CT表現(xiàn)與部分脾臟炎性假瘤相似,確診需鏡檢及免疫組織化學(xué)檢查。
[Abstract]:Objective: to study the CT findings and pathological features of spleen inflammatory pseudotumor-like follicular dendritic cell tumor. Methods: the clinical features, CT findings and pathological data of 2 cases of inflammatory pseudotumoral follicular dendritic cell tumor confirmed by operation and pathology were analyzed retrospectively. the CT and pathological features of inflammatory pseudotumoral follicular dendritic cell tumor were summarized. Results: two cases of inflammatory pseudotumor-like follicular dendritic cell tumor showed circular low density mass on CT plain scan, with clear boundary and uniform density. The edge of the lesion was enhanced in the arterial phase of enhanced scan, and the lesion in portal vein phase showed progressive continuous enhancement and filling into the center. Pathology: inflammatory pseudotumor-like follicular dendritic cell tumor, in addition to fusiform cells and inflammatory cells, there are many granulomatous changes with central necrosis, which are composed of many nuclear giant cells and tissue cells. EBER in situ hybridization: EBER-ISH (). Conclusion: CT dynamic contrast-enhanced scan of inflammatory pseudotumor-like follicular dendritic cell tumor of spleen has the characteristics of gradual and continuous enhancement. The CT findings of inflammatory pseudotumor of spleen are similar to those of some inflammatory pseudotumor of spleen. Microscopic examination and histochemical examination are needed for the diagnosis of inflammatory pseudotumor of spleen.
