迭代算法改善不同BMI患者雙源CT腹部低劑量掃描圖像質(zhì)量的研究

【摘要】:目的:探討迭代重建算法(SAFIRE)對(duì)在通過降低管電壓來實(shí)現(xiàn)的腹部低劑量掃描患者圖像質(zhì)量的改善情況和劑量的降低程度。材料和方法:將294例腹部增強(qiáng)掃描患者(男174例、女120)根據(jù)隨機(jī)原則按掃描管電壓的高低(120KV掃描、100KV掃描)和BMI指數(shù)(標(biāo)準(zhǔn)體重18.5 kg/m2≤BMI24 kg/m2、超重24 kg/m2≤BMI28 kg/m2)分成四組:A組(120KV掃描、標(biāo)準(zhǔn)體重)、B組(100KV掃描、標(biāo)準(zhǔn)體重)、C組(120KV掃描、超重)、D組(100KV掃描、超重)。記錄四組的輻射劑量CTDIvol、DLP,計(jì)算ED。掃描后四組動(dòng)脈期和靜脈期均通過濾波反投影(FBP)法重建圖像,B組、D組增加迭代(SAFIRE)法重建圖像。對(duì)所有圖像由兩位高年資影像診斷醫(yī)生進(jìn)行評(píng)分(1—5分),測(cè)量所有病人動(dòng)脈期和靜脈期肝門層面肝臟、豎脊肌和動(dòng)脈期腹主動(dòng)脈、靜脈期門靜脈的CT值和標(biāo)準(zhǔn)差,測(cè)量皮下脂肪的標(biāo)準(zhǔn)差作為圖像噪聲。計(jì)算SNR和CNR,計(jì)算公式為SNR臟器=ROI臟器/SD臟器;CNR臟器=(ROI臟器-ROI肌肉)/SD脂肪。比較A組和B組、C組和D組的輻射劑量ED,將A組、B組和C組、D組FBP重建的圖像進(jìn)行獨(dú)立樣本比較,將B組和D組FBP重建圖像和SAFIRE重建圖像進(jìn)行配對(duì)樣本比較,比較內(nèi)容為:動(dòng)脈期和靜脈期的主觀評(píng)分、圖像噪聲、腹主動(dòng)脈、門靜脈、肝臟的SNR和CNR。結(jié)果:輻射劑量:B組、D組的CTDIvol、DLP、ED明顯低于A組、C組。主觀評(píng)價(jià):在動(dòng)脈期FBP重建和靜脈期FBP重建圖像,A組、B組無差異;C組優(yōu)于D組;B組、D組無論在動(dòng)脈期還是靜脈期,其SAFIRE重建圖像優(yōu)于FBP重建圖像?陀^評(píng)價(jià):在FBP重建時(shí)B組、D組的圖像噪聲高于A組、C組;B組、D組使用SAFIRE重建圖像噪聲低于使用FBP重建圖像噪聲。FBP重建動(dòng)脈期圖像的A、B組的SNR腹主動(dòng)脈以及C、D組的CNR腹主動(dòng)脈、CNR肝臟無統(tǒng)計(jì)學(xué)差異;A組的SNR肝臟優(yōu)于B組,B組CNR腹主動(dòng)脈、CNR肝臟的優(yōu)于A組,C組的SNR腹主動(dòng)脈、SNR肝臟優(yōu)于D組。靜脈期圖像A、B組的SNR門靜脈,C、D組CNR門靜脈、CNR肝臟無統(tǒng)計(jì)學(xué)差異;A組的SNR肝臟優(yōu)于B組,B組的CNR門靜脈、CNR肝臟優(yōu)于A組,C組的SNR門靜脈、SNR肝臟優(yōu)于D組。B組、D組SAFIRE重建圖像的信噪比和對(duì)比噪聲比無論動(dòng)脈期還是靜脈期均明顯優(yōu)于FBP重建。結(jié)論:標(biāo)準(zhǔn)體重患者和超重患者腹部增強(qiáng)CT檢查時(shí)都可以通過降低管電壓來獲得低劑量掃描;超重患者降低管電壓在常規(guī)方式重建所得的圖像質(zhì)量會(huì)下降,可采用迭代重建算法(SAFIRE)加以彌補(bǔ)。
