【摘要】：目的探討頭顱CT漩渦征對(duì)創(chuàng)傷性硬膜外出血血腫擴(kuò)大的預(yù)測(cè)價(jià)值。方法選擇本院急診創(chuàng)傷性硬膜外血腫患者75例,于傷后6h內(nèi)完成頭顱CT檢查,根據(jù)有、無(wú)漩渦征分為漩渦征陽(yáng)性組、陰性組,發(fā)病后24h或病情惡化時(shí)常規(guī)復(fù)查頭顱CT,依據(jù)多田公式計(jì)算硬膜外血腫量,比較分析兩組患者血腫擴(kuò)大的發(fā)生率。結(jié)果 75例患者中,CT發(fā)現(xiàn)漩渦征陽(yáng)性組、陰性組分別為40%(30/75)、60%(45/75),血腫擴(kuò)大占37.3%(28/75),其中,漩渦征陽(yáng)性組為89.3%(25/28),陰性組為10.7%(3/45),兩組患者血腫擴(kuò)大發(fā)生率比較差異有統(tǒng)計(jì)學(xué)意義(χ2=25.72,P0.01)。結(jié)論頭顱CT漩渦征可作為預(yù)測(cè)急性創(chuàng)傷性硬膜外血腫擴(kuò)大的影像指標(biāo)。
[Abstract]:Objective to evaluate the predictive value of CT whirlpool sign in predicting the enlargement of traumatic extradural haematoma. Methods Seventy-five patients with traumatic epidural hematoma in our hospital were examined by CT within 6 hours after injury. They were divided into positive group and negative group according to their existence and no whirlpool sign. 24 hours after onset or when the condition worsened, the brain CT, was routinely reexamined. According to the multi-field formula, the epidural hematoma volume was calculated and the incidence of hematoma enlargement was compared between the two groups. Results of the 75 patients, 40% (30 / 75) and 60% (45 / 75) were found vortex sign in CT positive group, 37.3% (28 / 75) in hematoma enlargement group, 89.3% (25 / 28) in whirlpool positive group, 30% (30 / 75) in negative group, 60% (45 / 75) in negative group, and 87.3% (25 / 28) in whirlpool sign positive group, respectively. The incidence of hematoma enlargement in the negative group was 10.7% (3 / 45), and there was a significant difference between the two groups (蠂 2, 25.72, P0.01). Conclusion CT whirlpool sign can be used as an imaging index to predict the enlargement of acute traumatic epidural hematoma.
