【摘要】：目的通過(guò)早產(chǎn)兒尸體標(biāo)本Vater壺腹內(nèi)灌注對(duì)比劑,64-MSCT顯示胰膽管及其周?chē)M織結(jié)構(gòu),為進(jìn)一步研究提供依據(jù)。方法通過(guò)對(duì)15例不同胎齡早產(chǎn)兒尸體解剖十二指腸乳頭后,采用插管法對(duì)十二指腸乳頭肌內(nèi)灌注對(duì)比劑后,行64-MSCT掃描并結(jié)合三維重建,所得圖像由兩位高年資醫(yī)師獨(dú)立閱片并達(dá)成一致意見(jiàn),將十二指腸乳頭肌和胰膽管分別取材,進(jìn)行HE染色。結(jié)果共15例早產(chǎn)兒組織學(xué)標(biāo)本,9例胰膽管同時(shí)顯影,可見(jiàn)共同管,5例膽總管顯影,1例胰管顯影。胰膽管合流方式:Y型11例,U型2例,V型1例,Ⅱ型1例。乳頭肌形態(tài)及位置:乳頭肌有9例呈半球狀、3例呈扁平狀、3例呈柱狀;2例發(fā)現(xiàn)有小乳頭。9例乳頭位于十二指腸中部1/3,2例位于上部1/3,3例位于下部1/3,1例位于十二指腸遠(yuǎn)端。結(jié)論經(jīng)早產(chǎn)兒標(biāo)本乳頭肌內(nèi)灌注對(duì)比劑后64-MSCT結(jié)合三維重建可以清晰顯示胰膽管合流方式、共同管和周?chē)M織結(jié)構(gòu)。
[Abstract]:Objective to display the pancreatic bile duct and its surrounding tissue structure by Vater intraampullary perfusion contrast medium in cadaveric specimens of premature infants, so as to provide a basis for further study. Methods after dissecting duodenal papillae in 15 preterm infants of different gestational ages, intramuscular perfusion of contrast agent was performed by catheterization, then 64-MSCT scanning and three-dimensional reconstruction were performed. The images were read independently by two senior doctors and agreed. The duodenal papillary muscle and pancreaticobiliary duct were collected and stained with HE. Results in 15 preterm infants, 9 cases had simultaneous cholangiopancreatography, 5 cases had common duct and 1 case had pancreatic duct. There were 11 cases of Y-type, 2 cases of U-type, 1 case of V-type and 1 case of type 鈪，
