OsiriX測(cè)量感染性角膜炎的光學(xué)相干斷層圖像的可行性分析

【摘要】:目的 分析OsiriX用來測(cè)量感染性角膜炎在眼前段光學(xué)相干斷層成像(AS-OCT, anterior segment optical coherence tomography)的可行性,測(cè)量指標(biāo)包括浸潤(rùn)部位角膜厚度,浸潤(rùn)面積以及與浸潤(rùn)部位密度變化相關(guān)的值。 資料與方法 回顧性分析2011年12月至2012年3月在我院診斷為感染性角膜炎的病人的32張AS-OCT圖片。首先比較Visante OCT機(jī)器自帶軟件所測(cè)量的浸潤(rùn)部位中心角膜厚度與OsiriX測(cè)量所得的浸潤(rùn)部位中心角膜厚度是否具有一致性。再分析OsiriX測(cè)量感染性角膜炎AS-OCT圖片上的浸潤(rùn)面積與OCT值的可行性,評(píng)估指標(biāo)包括測(cè)量的可重復(fù)性和可再現(xiàn)性。Visante OCT與OsiriX兩種測(cè)量方法的一致性采用配對(duì)T檢查,Pearson線性相關(guān)分析,Bland-Altman分析法綜合分析。OsiriX測(cè)量感染性角膜炎AS-OCT圖片的可行性采用配對(duì)T檢查和組內(nèi)相關(guān)系數(shù)(intraclass correlation coefficients, ICC)綜合分析。以P0.05作為差異有統(tǒng)計(jì)學(xué)差異。 結(jié)果 1、Visante OCT與OsiriX測(cè)量所測(cè)量的浸潤(rùn)部位中心角膜厚度,平均值分別為700.19±171.67μm和694±166.84μm,差值的平均值為6.21±10.45μm,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。Pearson線性相關(guān)分析示兩種方法測(cè)量所得的浸潤(rùn)部位中心角膜厚度具有高度的相關(guān)性(相關(guān)系數(shù)r為0.998,P0.05)。由Bland—Altman分析法示僅3.1%(1/32)在95%一致性區(qū)間之外,兩者的一致性較好。 2、OsiriX測(cè)量感染性角膜炎浸潤(rùn)病灶面積,同一操作者兩次測(cè)量的結(jié)果平均值分別是2207641.94±1196023.88μm2和2170665.94±1170061.01“m2,兩者結(jié)果無顯著性差異(P值0.05),具有良好的可重復(fù)性(ICC0.9)。兩個(gè)不同操作者測(cè)量結(jié)果的平均值分別是2207641.94±1196023.88μm2和2201682.04±1177914.01μm2,P值為0.780.05,結(jié)果無顯著性差異(P值0.05),具有良好的可再現(xiàn)性(ICC0.9)。 3、OsiriX測(cè)量感染性角膜炎浸潤(rùn)病灶OCT值,同一操作者兩次測(cè)量的結(jié)果平均值分別是368.38±135.22和376.72±133.54,結(jié)果無顯著性差異(P值0.05),具有良好的可重復(fù)性(ICC0.9)。兩個(gè)不同操作者兩次測(cè)量的結(jié)果平均值分別是368.38±135.22和371.63±130.06,結(jié)果無顯著性差異(P值0.05),具有良好的可再現(xiàn)性(ICC0.9)。 結(jié)論 OsiriX測(cè)量感染性角膜炎浸潤(rùn)病灶中心部位角膜厚度與Visante OCT機(jī)器自帶的軟件具有良好的一致性,其測(cè)量不規(guī)則病灶的面積的功能以及和病灶密度相關(guān)的OCT值的功能具有較好的可重復(fù)性和可再現(xiàn)性,因此認(rèn)為OsiriX能夠用于測(cè)量感染性角膜炎AS-OCT的圖片。
[Abstract]:Objective to analyze the feasibility of using OsiriX to measure the corneal thickness of infective keratitis in anterior segment optical coherence tomography (AS-OCT, anterior segment optical coherence tomography). The infiltration area and the value related to the change of the density of the infiltrating site. Materials and methods 32 AS-OCT images of patients with infectious keratitis from December 2011 to March 2012 were analyzed retrospectively. The thickness of the central cornea of the infiltrating site measured by Visante OCT software was compared with that of the central corneal thickness of the infiltrating site measured by OsiriX. The feasibility of OsiriX in measuring the area of infiltration and the value of OCT on AS-OCT images of infectious keratitis was analyzed. The evaluation indexes included reproducibility and reproducibility of. Visante OCT and OsiriX. Pearson linear correlation analysis and Bland-Altman analysis were used to analyze the feasibility of OsiriX in the measurement of AS-OCT images of infectious keratitis by paired T test and (intraclass correlation coefficients, ICC) comprehensive analysis of intragroup correlation coefficient. With P0.05 as the difference was statistically significant. Results 1Visante OCT and OsiriX were used to measure the central corneal thickness of the infiltrating site. The mean values were 700.19 鹵171.67 渭 m and 694 鹵166.84 渭 m, respectively, and the difference was 6.21 鹵10.45 渭 m. The difference was statistically significant (P0.05). Pearson linear correlation analysis showed that there was a high correlation between the central corneal thickness measured by the two methods (r = 0.998 P 0.05). Bland-Altman analysis shows that only 3.1% (1 / 32) is outside the 95% consistency range. 2OsiriX was used to measure the area of infectious keratitis. The mean values of the two measurements were 2207641.94 鹵1196023.88 渭 m ~ 2 and 2170665.94 鹵1170061.01 "m ~ 2, respectively. There was no significant difference between the two results (P < 0.05). It has good repeatability (ICC0.9). The average values of two different operators were 2207641.94 鹵1196023.88 渭 m ~ 2 and 2201682.04 鹵1177914.01 渭 m ~ 2, P = 0.780.05.The results showed no significant difference (P < 0.05) and had good reproducibility (ICC0.9). 3OsiriX was used to measure the OCT value of infectious keratitis infiltrating foci. The mean values of the two measurements of the same operator were 368.38 鹵135.22 and 376.72 鹵133.54, respectively. There was no significant difference in the results between the two groups (P < 0. 05). It has good repeatability (ICC0.9). The mean values of the two measurements were 368.38 鹵135.22 and 371.63 鹵130.06, respectively. Conclusion the measurement of corneal thickness at the center of infectious keratitis by OsiriX is in good agreement with the software of Visante OCT machine. The function of measuring the area of irregular lesions and the OCT value related to the density of the lesion is reproducible and reproducible. It is considered that OsiriX can be used to measure the AS-OCT images of infectious keratitis.
