256CT低管電壓聯(lián)合迭代重組技術在頭頸部CT血管成像中的應用

【摘要】:目的探討低管電壓(80 k V)聯(lián)合迭代重組技術在頭頸部CT血管成像(CTA)掃描中對圖像質量、輻射劑量的影響。方法將75例臨床需要行頭頸部CTA檢查的患者隨機編入A、B、C三組,每組25例。A組為常規(guī)劑量掃描,參數(shù)為120 k V、300 m As,經濾波反投影(FBP)重組;B、C組為低劑量掃描,管電流和管電壓分別為120k V、180 m As,80 k V、180 m As,采用i Dose4迭代重組。對比分析CT圖像質量,包括動脈強化值、對比噪聲比(CNR)、圖像質量評分及輻射劑量。計量資料采用配對t檢驗兩兩比較,圖像質量評分采用非參數(shù)χ2檢驗進行統(tǒng)計分析。結果 (1)圖像質量主觀評分:A組3分1例,4分24例;B組3分2例,4分23例;C組3分5例,4分20例;統(tǒng)計學分析無差異(P㧐0.05)。(2)A、B兩組主動脈弓、頸總動脈、頸內動脈及大腦中動脈的動脈強化CT值統(tǒng)計無明顯差異(P㧐0.05);C組的CT動脈強化值高于A組(P0.05)。(3)C組的背景噪聲[(12.5±1.9)HU]高于A組[(9.0±1.6)HU](P0.05);兩組的CNR分別為(49.2±9.4)、(43.0±12.6),無統(tǒng)計學差異(P㧐0.05)。(4)A、B、C組的劑量長度乘積(DLP)分別為(925.1±48.5)、(571.4±24.2)、(164.1±7.0)m Gy·cm;輻射有效劑量(ED)分別為(2.87±0.15)、(1.77±0.08)、(0.51±0.02)m Sv,均有統(tǒng)計學意義(P0.05),C組較A組輻射劑量明顯降低,約82.3%。結論 256 CT頭頸部CTA采用低管電壓(80 k V)聯(lián)合迭代重組技術能提供滿足臨床診斷要求的圖像,同時明顯降低輻射劑量。
[Abstract]:Objective to investigate the effect of low tube voltage (80 k V) combined with iterative recombination) on image quality and radiation dose in CT angiography (CTA) scanning of head and neck. Methods Seventy-five patients with head and neck CTA were randomly assigned to group C (25 cases in each group). Group A was scanned with conventional dose and the parameters were 120kV / 300m As, reconstructed by filtered backprojection (FBP). The current and voltage of the tube were 120kV ~ (180 m) As,80 / k V ~ (180) m As, respectively. I Dose4 was used to iterate the recombination of the current and the voltage of the tube. The image quality of CT, including arterial enhancement value, contrast noise ratio (CNR),) image quality score and radiation dose, were compared and analyzed.
