【摘要】：目的探討和分析透明血管型Castleman病的CT表現(xiàn),提升診斷準(zhǔn)確率。方法回顧性分析2010年7月—2015年5月行CT雙期增強(qiáng)掃描并經(jīng)病理證實的透明血管型Castleman病的23例患者資料。結(jié)果23例透明血管型Castleman病中,21例增強(qiáng)掃描病灶呈顯著均勻強(qiáng)化,均分布在大血管旁,且被迂曲粗大血管包繞;1例病灶呈輕度均勻強(qiáng)化,分布在升結(jié)腸旁,周圍無大血管;另1例骨盆諸骨有成骨性和溶骨性骨質(zhì)破壞。23例中有4例含鈣化灶,其中1例CT表現(xiàn)為中央?yún)^(qū)星芒狀鈣化灶,3例邊緣分別呈線狀、弧線和點狀鈣化灶。1例病灶位于左頸部,手術(shù)切除后1年原手術(shù)區(qū)復(fù)發(fā)。結(jié)論透明血管型Castleman病腫瘤位于大血管旁并被粗大迂曲血管包繞者呈顯著強(qiáng)化,CT動靜脈雙期呈持續(xù)性顯著均勻強(qiáng)化,鈣化的特征對透明血管型Castleman病的診斷具有重要價值。成骨性和溶骨性骨質(zhì)破壞,局限性透明血管型Castleman病手術(shù)切除后復(fù)發(fā)屬罕見表現(xiàn)。
[Abstract]:Objective to investigate and analyze the CT features of transparent vascular type Castleman disease and to improve the diagnostic accuracy. Methods from July 2010 to May 2015, 23 patients with transparent vascular Castleman's disease were retrospectively analyzed. Results among 23 cases of transparent vascular type Castleman's disease, 21 cases showed homogeneous enhancement in contrast enhancement, all of them were distributed around the large vessels, and 1 case showed slight homogeneous enhancement, which was distributed beside the ascending colon, and no large vessels were found around the lesions. In the other case, osteogenic and osteolytic bone destruction was found in the pelvic bones. 4 of the 23 cases had calcification foci, of which 1 case showed starlike calcification in the central region and 3 cases showed linear margin. One case was located in the left neck and recurred 1 year after resection. Conclusion the tumor of transparent vascular type Castleman's disease is located next to the large vessel and surrounded by large and tortuous vessels, and the CT arteriovenous dual phase is continuously and uniformly enhanced. Calcification is of great value in the diagnosis of transparent vascular type Castleman's disease. Osteogenic and osteolytic bone destruction and recurrence of local transparent vascular Castleman's disease are rare.
