婦科腫瘤后裝逆向調(diào)強(qiáng)放射治療的劑量學(xué)研究

【摘要】:放射治療在婦科腫瘤治療中占有重要的地位。隨著逆向調(diào)強(qiáng)放射治療技術(shù)在外照射治療中的廣泛應(yīng)用,其優(yōu)越性已被大量實(shí)踐證明,但該技術(shù)在內(nèi)照射治療中應(yīng)用卻很少。本研究通過(guò)探討后裝逆向調(diào)強(qiáng)放射治療在婦科腫瘤治療中的應(yīng)用價(jià)值,為臨床治療的技術(shù)選擇提供依據(jù)。 本研究選取20例Ⅰ~Ⅲ期宮頸癌患者,隨機(jī)分為A、B兩組,各10例。Ⅰb期2例,Ⅱa期4例,Ⅱb期12例,Ⅲa期2例。病理分型:鱗癌16例,腺癌4例。以上診斷均經(jīng)過(guò)病理學(xué)證實(shí)。平均年齡55.67歲(32~65歲)。A組病例行后裝逆向調(diào)強(qiáng)放射治療,采用Inverse Planning Simulated Annealing(IPSA)技術(shù)進(jìn)行劑量?jī)?yōu)化設(shè)計(jì),危及器官(直腸、膀胱)進(jìn)行劑量限制優(yōu)化;B組病例行三維適形后裝放射治療,危及器官(直腸、膀胱)不予以劑量限制優(yōu)化。A、B組患者均采用3根施源器,,進(jìn)行腔內(nèi)照射并同步箱式四野等中心外照射,劑量為46Gy/23次。依據(jù)曼徹斯特劑量學(xué)系統(tǒng),給予A點(diǎn)(宮口上2cm,宮腔中軸線左右旁開2cm處為A點(diǎn)位置)處方劑量6Gy/次,照射次數(shù)5次,2次/周。 通過(guò)對(duì)比分析A、B兩組患者的靶區(qū)劑量分布,危及器官(直腸、膀胱)受量,放射性并發(fā)癥及近期療效。結(jié)果顯示: A、B兩組患者的靶區(qū)劑量均能達(dá)到處方劑量要求,A組患者的靶區(qū)劑量適形度和均勻性均好于B組患者。A組患者的0.5~1年的腫瘤局部和區(qū)域控制率可達(dá)95%以上。危及器官(直腸、膀胱)受量:A組患者的直腸、膀胱最大受量比B組患者的直腸、膀胱受量小約35%。A組患者的直腸、膀胱75%處方劑量受照體積約只為B組患者的二分之一。放射性并發(fā)癥:A組患者發(fā)生RTOG1級(jí)反應(yīng)為0例,B組患者發(fā)生RTOG1級(jí)反應(yīng)為6例。表現(xiàn)為大便次數(shù)增加,黏液便,或尿頻尿急尿痛。A組患者發(fā)生RTOG2級(jí)反應(yīng)為0例,B組患者發(fā)生RTOG2級(jí)反應(yīng)為3例。表現(xiàn)為腹瀉,伴或不伴黏液分泌增多,尿頻、尿急、排尿困難及膀胱痙攣痛,需局部用藥。近期療效:在A、B組患者放療期間及治療結(jié)束后,幾乎所有患者的腫瘤均有不同程度的縮小,有效率(CR+PR)均為92%以上。 研究結(jié)論表明,后裝逆向調(diào)強(qiáng)放射治療技術(shù)具有近期療效明顯,靶區(qū)劑量適形度、均勻性好,危及器官受量低,放射性并發(fā)癥少等優(yōu)點(diǎn),值得在婦科腫瘤治療中廣泛應(yīng)用。
