【摘要】：目的人工耳蝸植入術(shù)(CI)前需評價(jià)內(nèi)耳情況,嬰幼兒正常內(nèi)耳結(jié)構(gòu)的HRCT測量為評價(jià)內(nèi)耳發(fā)育提供影像依據(jù)。資料與方法前瞻性選取正常顳骨HRCT圖像129例(258耳),按年齡分為A組92例(184耳,1~12個(gè)月),B組21例(42耳,13~24個(gè)月),C組16例(32耳,25~36個(gè)月)。測量耳蝸頂中周高度、耳蝸底周長徑、耳蝸底周內(nèi)徑、耳蝸神經(jīng)管(CNC)寬徑、耳蝸導(dǎo)水管(CA)長徑、CA外口寬徑、外半規(guī)管(LSCC)骨島直徑,統(tǒng)計(jì)LSCC骨島中心透亮影的陽性率。結(jié)果不同年齡、性別和側(cè)別間的耳蝸底周長徑、管內(nèi)徑、頂中周高度、CNC寬徑及LSCC骨島寬徑相比,差異無統(tǒng)計(jì)學(xué)意義(P0.05);不同性別和側(cè)別間的CA長徑及外口寬徑相比,差異無統(tǒng)計(jì)學(xué)意義(P0.05),但隨年齡增長而增大。不同性別和側(cè)別間的LSCC骨島中心透亮影陽性率相比,差異無統(tǒng)計(jì)意義(P0.05),在A組的發(fā)生率最高,陽性率隨年齡增大而減小。結(jié)論嬰幼兒正常內(nèi)耳結(jié)構(gòu)的HRCT測量有助于了解內(nèi)耳結(jié)構(gòu)的發(fā)育特點(diǎn),為CI前評價(jià)內(nèi)耳情況提供參考依據(jù)。
[Abstract]:Objective to evaluate the condition of inner ear before cochlear implantation (CI), and to evaluate the development of inner ear by HRCT measurement of normal inner ear structure in infants. Materials and methods 129 patients (258 ears) with normal temporal bone HRCT images were prospectively divided into group A (n = 92) (184 ears), B group (n = 21) 1 ~ 12 months (n = 21), C group (n = 16) (n = 16, n = 32, 25 ~ 36 months). The median height of cochlea, the diameter of basal circumference of cochlea, the inner diameter of basal pericochlea, the (CNC) width of cochlear canal, the long diameter of cochlear aqueduct (CA), the diameter of CA outer mouth and the diameter of (LSCC) island of outer semicircular canal were measured. Results there was no significant difference in cochlear basal circumference diameter, inner diameter of cochlea, height of middle parietal circumference, CNC width and LSCC bone island width in different age, sex and lateral sex (P0.05). The difference was not statistically significant (P0.05), but increased with age. There was no statistical significance between the positive rate of LSCC bone island center transparency in different sex and side (P0.05). The positive rate was the highest in group A, and the positive rate decreased with the increase of age. Conclusion HRCT measurement of normal inner ear structure in infants is helpful to understand the developmental characteristics of inner ear structure and provide reference for evaluating inner ear condition before CI.
