【摘要】：目的對(duì)比分析不可逆電穿孔(IRE)及冷凍治療對(duì)豬肝實(shí)質(zhì)的消融效果。方法將4只實(shí)驗(yàn)用小型豬,隨機(jī)分為2組,每組2只。第1組先于CT引導(dǎo)下采用IRE及冷凍治療分別進(jìn)行肝臟消融,消融后常規(guī)飼養(yǎng);30天后以同法消融第2組小型豬,術(shù)中及術(shù)后對(duì)小型豬行螺旋CT平掃及增強(qiáng)掃描,掃描完成后處死兩組實(shí)驗(yàn)動(dòng)物并取出肝臟,采用10%甲醛溶液固定,將標(biāo)本整體切片行病理學(xué)檢查。結(jié)果術(shù)中CT平掃顯示冷凍消融區(qū)域邊界清晰,IRE消融邊界不清;術(shù)后增強(qiáng)CT掃描顯示IRE及冷凍消融區(qū)域術(shù)后即刻均未見(jiàn)強(qiáng)化。大體標(biāo)本:IRE消融區(qū)域邊界清晰,血管、膽管結(jié)構(gòu)良好;冷凍消融區(qū)域邊界模糊,局部可見(jiàn)出血。結(jié)論 IRE及冷凍治療對(duì)豬肝組織消融效果良好,而IRE對(duì)肝內(nèi)血管、膽管具有保護(hù)作用,可用于肝門、大血管區(qū)域的消融。
[Abstract]:Objective to compare and analyze the ablation effect of irreversible electroporation (IRE) and cryotherapy on porcine liver parenchyma. Methods four experimental miniature pigs were randomly divided into 2 groups. The first group was treated with IRE and cryotherapy under the guidance of CT respectively. After 30 days of ablation, the second group was ablated with the same method. Spiral CT plain scan and enhanced scanning were performed during and after operation. After scanning, the two groups of experimental animals were killed and the liver was removed, fixed with 10% formaldehyde solution, and the whole specimen was sectioned for pathological examination. Results the boundary of cryoablation area was clearly defined by CT plain scan during operation and the boundary of ire ablation was not clear by contrast enhanced CT scan, neither IRE nor cryoablation area was enhanced immediately after operation. The boundary of the ablation area was clear, the blood vessel and bile duct were well constructed, the boundary of the frozen ablation area was blurred, and the local bleeding was observed. Conclusion IRE and cryotherapy have a good effect on the ablation of porcine liver tissue, while IRE has protective effect on intrahepatic vessels and bile ducts, and can be used in the ablation of hepatic hilus and large vascular regions.
