256層CT全肝灌注評(píng)估肝硬化段性功能儲(chǔ)備

【摘要】：目的:以臨床Ch ild-pug h分級(jí)為標(biāo)準(zhǔn),應(yīng)用256層CT全肝灌注掃描對(duì)肝硬化患者行段性功能儲(chǔ)備評(píng)估。方法:20例無(wú)明顯肝臟疾患的患者及23例肝硬化患者均行全肝灌注檢查,得到肝臟灌注圖像,分別測(cè)量肝臟各段灌注參數(shù):肝動(dòng)脈灌注量(HAP)、門靜脈灌注量(HPP)、肝總灌注量(TLP)、肝動(dòng)脈灌注指數(shù)(HAPI)以及門靜脈灌注指數(shù)(HPPI)。對(duì)正常組及肝硬化組行肝段性灌注測(cè)量,對(duì)肝臟各段血流灌注參數(shù)行統(tǒng)計(jì)學(xué)分析。結(jié)果:正常肝臟各段間HAP、HPP、TLP、HAPI、HPPI均無(wú)統(tǒng)計(jì)學(xué)差異,HAP:HPP=1/3~1/4;Ch ild-pug h A級(jí)H A P、T LP存在段間差異(P0.05),H A P:H PP=1/2~1/3;Ch ild-pug h B級(jí)H A P存在段間差異(P0.05),HAP:HPP=1/2~1/3;Child-pugh C級(jí)各灌注參數(shù)均存在段間差異(P0.05),HAP:HPP=2/1~2/3。正常組與肝硬化各組間比較:除Ⅱ、Ⅲ段HAP組間無(wú)統(tǒng)計(jì)學(xué)差異外,Ⅰ-Ⅷ段各灌注參數(shù)均存在部分組間差異(P0.05),Ⅰ-Ⅷ段HPP肝硬化各組與正常組均有統(tǒng)計(jì)學(xué)差異,隨著肝功能受損程度的加重,肝臟各段HAP、HAPI值呈上升趨勢(shì),HPP、TLP、HPPI值呈下降趨勢(shì),肝動(dòng)脈與門靜脈系統(tǒng)血供比例逐漸增大。結(jié)論:256層CT全肝灌注模式可用于直觀定量反映肝硬化肝臟各段血流灌注特征,對(duì)肝臟儲(chǔ)備功能分段評(píng)估具有一定臨床應(yīng)用價(jià)值。
[Abstract]:Objective: to evaluate segmental functional reserve in patients with liver cirrhosis by 256-slice CT total hepatic perfusion scan according to clinical Ch ild-pug h grade. Methods Twenty patients without obvious liver disease and 23 patients with liver cirrhosis were examined with whole liver perfusion, and liver perfusion images were obtained. Hepatic perfusion parameters: hepatic artery perfusion (HAP), portal vein perfusion (HPP), total hepatic perfusion (TLP), hepatic artery perfusion index (HAPI) and portal vein perfusion index (HPPI). Hepatic segmental perfusion was measured in normal group and liver cirrhosis group. Results: there was no significant difference in HAP,HPP,TLP,HAPI,HPPI between normal liver segments (P 0.05).
