【摘要】：目的:分析兒童肝臟脈管性病變的影像學(xué)特征,復(fù)習(xí)國(guó)際脈管性病變研究協(xié)會(huì)(ISSVA)脈管性病變分類及命名標(biāo)準(zhǔn),明確影像診斷及正確命名在肝臟脈管性病變?cè)\斷中的重要性。方法:搜集本院2006年1月-2014年12月91例兒童肝臟脈管性病變患者的病例資料,其中3例經(jīng)手術(shù)證實(shí),48例經(jīng)隨訪證實(shí)。91例中男46例,女45例,年齡15天-11歲,中位年齡3歲。將影像診斷(超聲、CT、MRI)與術(shù)后病理結(jié)果以及隨訪資料進(jìn)行對(duì)照分析。結(jié)果:91例中89例未按照ISSVA標(biāo)準(zhǔn)正確命名;CT和MRI顯示肝臟脈管性病變表現(xiàn)為孤立性單發(fā)腫瘤57例,多發(fā)腫瘤21例,彌漫性腫瘤13例;病變的CT和MRI表現(xiàn)相似,均為動(dòng)脈期明顯強(qiáng)化,靜脈期持續(xù)強(qiáng)化,表現(xiàn)為"快進(jìn)慢出"的特點(diǎn);91例中4例合并Kasabach-Merritt綜合征。結(jié)論:肝臟脈管性病變亦需根據(jù)ISSVA標(biāo)準(zhǔn)進(jìn)行規(guī)范命名和分類,影像檢查是兒童肝臟脈管性病變的首選診斷方法。
[Abstract]:Objective: to analyze the imaging features of hepatic vascular lesions in children, review the classification and naming criteria of (ISSVA) vascular lesions of the International Association for the study of Vascular lesions, and clarify the importance of imaging diagnosis and proper naming in the diagnosis of hepatic vascular diseases. Methods: the data of 91 children with hepatic vascular diseases from January 2006 to December 2014 were collected. Among them, 48 cases were confirmed by operation. Among them, 46 cases were males and 45 cases were females, aged from 15 days to 11 years, with a median age of 3 years. Imaging diagnosis (CT MRI) was compared with postoperative pathological results and follow-up data. Results among 91 cases, 89 cases were not named correctly according to ISSVA standard. Ct and MRI showed that 57 cases of hepatic vascular lesions showed solitary tumors, 21 cases of multiple tumors and 13 cases of diffuse tumors. The CT and MRI findings of the lesions were similar. All of them showed obvious enhancement in arterial phase and continuous enhancement in venous phase, which were characterized by "fast in and slow out" in 4 out of 91 cases with Kasabach-Merritt syndrome. Conclusion: hepatic vascular lesions should also be named and classified according to ISSVA standard. Imaging is the first choice for diagnosis of hepatic vascular diseases in children.
