雙能CT單能譜應(yīng)用對(duì)冠脈小內(nèi)徑支架顯示的影響

【摘要】:目的:評(píng)價(jià)雙能CT單能譜技術(shù)不同能級(jí)對(duì)3 mm及以下冠脈支架顯示的影響。方法:選取不同材質(zhì)7枚內(nèi)徑3 mm或2.5 mm支架,模擬冠脈血管模型,行雙源CT雙能量冠脈掃描,將獲得的兩種不同能量(100及140 k V)數(shù)據(jù)采用單能譜技術(shù)重建處理,分別得到60~140 ke V(間隔10 keV)及線(xiàn)性融合120 kV 10組圖像;對(duì)上述10組圖像進(jìn)行雙盲主觀評(píng)價(jià)以及包括支架內(nèi)徑差異、支架內(nèi)腔衰減和圖像噪聲的客觀評(píng)價(jià),統(tǒng)計(jì)分析各組間差異性。結(jié)果:圖像質(zhì)量主觀評(píng)分一致性良好(Kappa=0.833),80 keV組獲得的圖像質(zhì)量總評(píng)分最高,為31分,除與90 ke V組及120 k V組差異未達(dá)到統(tǒng)計(jì)學(xué)意義外(P均0.05),與其他各組間均有顯著統(tǒng)計(jì)學(xué)差異(P值均0.05);80 keV組支架內(nèi)徑差異值最低,為36.1%±3.7%,明顯低于其他各能級(jí)組,差異均有統(tǒng)計(jì)學(xué)意義(P值0.05);各組間支架管腔內(nèi)衰減差異未達(dá)統(tǒng)計(jì)學(xué)差異(P=0.151)。80 keV組噪聲最低,為(12.51±1.27)HU,與其各組間均有統(tǒng)計(jì)學(xué)差異(P值0.05)。結(jié)論:對(duì)于3 mm及以下支架的評(píng)估,80 keV能級(jí)圖像重組獲得的圖像質(zhì)量最佳,更符合臨床工作需求,值得推薦使用。
[Abstract]:Objective: to evaluate the effects of different energy levels of dual energy CT single energy spectroscopy on the display of coronary stents under 3 mm and below. Methods: seven stents with 3 mm or 2.5 mm inner diameter of different materials were selected to simulate the coronary artery model. The two different energy sources (100 and 140 k V) were reconstructed by single energy spectrum technique. 10 groups of images with 60 ke V (interval of 10 keV) and 10 groups of linear fusion images were obtained, and the subjective evaluation of double-blind and objective evaluation including the difference of stent diameter, the attenuation of stent cavity and the image noise were carried out, and the differences among the 10 groups were statistically analyzed. Results: the total score of image quality was the highest in the group of 80 keV with good consistency of image quality (Kappa=0.833), with a score of 31. Except that there was no significant difference between 90keV group and 120kV group (P 0.05), there was significant difference (P 0. 05) between the other groups and other groups (P 0. 05). The difference of stent diameter in the 80 keV group was the lowest (36. 1% 鹵3. 7%), which was significantly lower than that in other energy level groups. The difference was statistically significant (P0. 05), and the difference in the attenuation of stent lumen was not up to statistical difference (P0. 151). 80 keV group had the lowest noise (12. 51 鹵1. 27) HUU, and there was statistical difference between each group (P 0. 05). Conclusion: the reconstruction of 80 keV energy level images with 3 mm or less scaffolds has the best image quality, which is more suitable for clinical work and is worthy of recommended use.
