【摘要】：目的探討胸部CT主肺動脈比(RAP)與血壓的相關(guān)性及臨床意義。方法回顧性分析191例高血壓患者(高血壓組)及87名健康體檢者(對照組)的胸部CT資料。高血壓組根據(jù)血壓升高情況分為T1組(1級高血壓,63例)、T2組(2級高血壓,60例)及T3組(3級高血壓,68例),根據(jù)治療情況分為A組(治療組,94例)和B組(未治療組,97例);結(jié)合血壓和治療情況,其中A1組31例、A2組30例、A3組33例,B1組32例、B2組30例、B3組35例。對照組根據(jù)年齡分為C1組(老年組,年齡≥60歲)40名及C2組(非老年組,年齡60歲)47名。通過平掃軸位圖像測量升主動脈、肺動脈直徑,計算RAP。連續(xù)7天記錄清晨穩(wěn)定血壓并取平均值作為臨床血壓檢測結(jié)果,包括收縮壓(SBP)及舒張壓(DBP)。結(jié)合年齡、血壓等級及治療情況分組分析血壓與RAP的相關(guān)性,并評價RAP對1級高血壓的診斷效能。結(jié)果 T1、T2、T3、C1及C2組間兩兩比較,SBP及RAP差異均有統(tǒng)計學(xué)意義(P均0.05),DBP差異均無統(tǒng)計學(xué)意義(P均0.05)。A1、A2、A3、B1、B2及B3組間兩兩比較,升主動脈、肺動脈直徑及RAP差異均有統(tǒng)計學(xué)意義(P均0.05)。對照組、高血壓組、A組、B組SBP均與RAP呈正相關(guān)(P均0.05)。對照組及高血壓組DBP與RAP均無相關(guān)性(P均0.05)。以RAP=1.29為閾值,診斷1級高血壓的敏感度為85.71%(54/63),特異度為89.66%(78/87),誤診率10.34%(9/87),漏診率14.29%(9/63),Youden指數(shù)為0.75。結(jié)論胸部CT所測RAP可客觀反映血壓水平,RAP與SBP呈正相關(guān)。
[Abstract]:Objective to investigate the correlation and clinical significance between (RAP) and blood pressure in chest CT. Methods the chest CT findings of 191 patients with hypertension (hypertension group) and 87 healthy persons (control group) were analyzed retrospectively.
