ER和PR同一表達(dá)狀態(tài)下HER-2與乳腺癌X線表現(xiàn)的相關(guān)性分析

【摘要】:目的探討乳腺癌免疫組織化學(xué)中雌激素受體(ER)和孕激素受體(PR)表達(dá)一致條件下,人類表皮生長(zhǎng)因子受體2(HER-2)的表達(dá)狀態(tài)和乳腺癌X線表現(xiàn)之間的相關(guān)性。方法回顧性分析經(jīng)手術(shù)病理確診的1533例女性乳腺癌患者的X線表現(xiàn),根據(jù)免疫組織化學(xué)(IHC)中HER-2、ER、PR的表達(dá)結(jié)果分為Ⅰa(HER-2陽(yáng)性/ER-/PR-)和Ⅰb(HER-2陰性/ER-/PR-)、Ⅱa(HER-2陽(yáng)性/ER+/PR+)和Ⅱb(HER-2陰性/ER+/PR+)兩個(gè)配對(duì)組。分別對(duì)兩個(gè)配對(duì)組中乳腺癌的腫塊和鈣化進(jìn)行分析,并對(duì)各項(xiàng)分析結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)檢驗(yàn)。結(jié)果Ⅰa和Ⅰb組中,HER-2表達(dá)陽(yáng)性乳腺癌多見(jiàn)單純鈣化(18.3%)、腫塊伴鈣化(23.7%),HER-2表達(dá)陰性乳腺癌單純腫塊比較多見(jiàn)(49.2%);HER-2表達(dá)陽(yáng)性乳腺癌腫塊形態(tài)以不規(guī)則形多見(jiàn)(47.4%),HER-2表達(dá)陰性乳腺癌腫塊以類圓形(58.8%)、分葉狀(12.1%)多見(jiàn);HER-2表達(dá)陽(yáng)性乳腺癌腫塊邊界多模糊不清(86.1%),HER-2表達(dá)陰性乳腺癌腫塊邊界多清楚局限(9.8%)。與HER-2表達(dá)陽(yáng)性乳腺癌比較,陰性乳腺癌砂礫狀鈣化(55.2%)多見(jiàn),且大多呈區(qū)域性分布(32.1%)。Ⅱa和Ⅱb組中,HER-2表達(dá)陽(yáng)性乳腺癌類圓形(52.1%)、分葉狀腫塊(11.0%)多見(jiàn),HER-2表達(dá)陰性乳腺癌不規(guī)則形腫塊(40.6%)多見(jiàn);HER-2表達(dá)陽(yáng)性乳腺癌腫塊邊界多模糊不清(90.7%),大多可見(jiàn)毛刺邊緣(66.7%),HER-2表達(dá)陰性乳腺癌腫塊邊緣多見(jiàn)淺分葉(65.3%)。與HER-2表達(dá)陰性乳腺癌比較,陽(yáng)性乳腺癌砂礫狀鈣化(68.4%)多見(jiàn),且大多呈節(jié)段性分布(31.9%),HER-2表達(dá)陰性乳腺癌鈣化大多成簇狀分布(57.6%)。結(jié)論各配對(duì)組中HER-2表達(dá)陽(yáng)性與陰性乳腺癌的腫塊及鈣化有著不同的X線表現(xiàn)。
