運(yùn)動(dòng)負(fù)荷對(duì)正常腰椎間盤(pán)MR擴(kuò)散張量成像表現(xiàn)彌散系數(shù)和各向異性分?jǐn)?shù)的影響

【摘要】:目的探討運(yùn)動(dòng)負(fù)荷對(duì)正常腰椎間盤(pán)MR擴(kuò)散張量成像(DTI)中表觀彌散系數(shù)(ADC)和部分各向異性(FA)的影響。方法對(duì)30例健康志愿者行常規(guī)矢狀位FSE/T2WI成像及運(yùn)動(dòng)負(fù)荷前后DTI成像,其中男性24名,女性6名,年齡19~25歲。在MR常規(guī)T2WI序列上對(duì)腰椎間盤(pán)按Pfirrmann標(biāo)準(zhǔn)進(jìn)行退變分級(jí),利用DTI原始數(shù)據(jù)重組b0圖、ADC圖及FA圖,測(cè)量并比較運(yùn)動(dòng)負(fù)荷前后ADC值、FA值的變化。統(tǒng)計(jì)方法使用重復(fù)測(cè)量數(shù)據(jù)的方差分析,P0.05具有統(tǒng)計(jì)學(xué)意義。結(jié)果 30名志愿者,觀測(cè)L1~S1共計(jì)150個(gè)椎間盤(pán),將其中27例腰椎間盤(pán)Pfirrminn分級(jí)均為Ⅱ級(jí)的受試者納入研究。腰椎間盤(pán)平均ADC值運(yùn)動(dòng)負(fù)荷前為(1.99±0.18)×10-3mm2/s,運(yùn)動(dòng)負(fù)荷后為(1.93±0.17)×10-3mm2/s,較運(yùn)動(dòng)負(fù)荷前下降,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);腰椎間盤(pán)平均FA值運(yùn)動(dòng)負(fù)荷前為0.155±0.059,運(yùn)動(dòng)負(fù)荷后為0.163±0.017,較運(yùn)動(dòng)負(fù)荷前升高,差異不具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論磁共振DTI成像可以定量分析運(yùn)動(dòng)負(fù)荷后腰椎間盤(pán)水分子擴(kuò)散能力及各向異性程度的變化。運(yùn)動(dòng)負(fù)荷后,腰椎間盤(pán)ADC值下降,FA值升高,其中ADC值的變化更為敏感。

Methods routine sagittal FSE/T2WI imaging and DTI imaging before and after exercise load were performed in 30 healthy volunteers, including 24 males and 6 females aged 19 to 25 years. The lumbar intervertebral disc was graded according to Pfirrmann standard on conventional Mr T2WI sequence. The changes of ADC value and FA value before and after exercise load were measured and compared by recombination of b0 map and FA map using DTI original data. Statistical method using repeated measurement data analysis of variance (P0.05) has statistical significance.
