【摘要】：目的:運(yùn)用多層螺旋CT血管成像(MSCTA)評(píng)價(jià)主動(dòng)脈壁內(nèi)血腫(IMH)及其預(yù)后,并經(jīng)Logistic回歸分析IMH預(yù)后因素。方法:回顧分析33例初次行胸腹主動(dòng)脈MSCTA診斷為IMH并進(jìn)行保守治療后再次MSCTA復(fù)查患者的臨床及影像資料,評(píng)估IMH的預(yù)后,并將患者初次行MSCTA時(shí)的年齡、性別、是否合并有粥樣硬化、Stanford分型、受累主動(dòng)脈直徑、血腫最大厚度是否≥11mm、是否合并穿透性動(dòng)脈粥樣硬化性潰瘍(PAU)、血腫內(nèi)是否有局限性強(qiáng)化影8項(xiàng)指標(biāo)納入Logistic回歸分析,探討IMH預(yù)后的危險(xiǎn)因素。結(jié)果:33例IMH患者中20例(60.6%)有進(jìn)展,經(jīng)Logistic回歸分析,得到的回歸方程為:Logit(P)=-10.489+1.915X1-2.277X3+3.251X5+3.436X6+2.018X7,年齡、受累主動(dòng)脈直徑、血腫最大厚度≥11mm及合并PAU是IMH預(yù)后不良的危險(xiǎn)因素,合并粥樣硬化為保護(hù)因素。結(jié)論:根據(jù)危險(xiǎn)因素和回歸預(yù)測(cè)結(jié)果,可對(duì)IMH預(yù)后做出較準(zhǔn)確的評(píng)價(jià),并對(duì)IMH的治療計(jì)劃選擇提供理論依據(jù),延長(zhǎng)患者生命。
[Abstract]:Objective: to evaluate aortic intramural hematoma (IMH) and its prognosis by multislice spiral CT angiography (MSCTA), and to analyze the prognostic factors of IMH by logistic regression analysis. Methods: the clinical and imaging data of 33 patients with IMH diagnosed as IMH by primary aortic MSCTA and conservative treatment were retrospectively analyzed.
