急性損傷后開始訓(xùn)練時(shí)間對(duì)髕骨髕腱結(jié)合部愈合的影響

【摘要】:背景:髕骨髕腱結(jié)合部損傷在運(yùn)動(dòng)訓(xùn)練中十分常見且復(fù)發(fā)率較高,及時(shí)合理的恢復(fù)訓(xùn)練對(duì)保持競(jìng)技狀態(tài)有利,而不恰當(dāng)?shù)幕謴?fù)訓(xùn)練則有可能加重?fù)p傷。目的:從組織形態(tài)學(xué)、生長(zhǎng)因子、膠原蛋白表達(dá)量及肌張力等角度,探討傷后休息不同天數(shù)恢復(fù)訓(xùn)練對(duì)骨腱結(jié)合部愈合質(zhì)量的影響。方法:40只新西蘭大白兔被隨機(jī)分為:空白對(duì)照組,針刺后分別休息24,48,72,96,120 h開始恢復(fù)訓(xùn)練組。除空白對(duì)照組外,其余各組于髕腱止點(diǎn)處進(jìn)行針刺損傷造模后采用已建立的循環(huán)載荷訓(xùn)練模型,進(jìn)行4周低強(qiáng)度訓(xùn)練后處死取材,行組織學(xué)蘇木精-伊紅、番紅及免疫組織化學(xué)染色;每次訓(xùn)練后利用Myoton-3肌肉功能分析儀采集肌張力,肌肉硬度數(shù)據(jù);Metamorph圖形處理軟件對(duì)各組纖維軟骨帶厚度、細(xì)胞密度,血管內(nèi)皮生長(zhǎng)因子、膠原陽(yáng)性染色細(xì)胞密度進(jìn)行定量計(jì)算。結(jié)果與結(jié)論:(1)與空白對(duì)照組相比,各傷后訓(xùn)練組在呈現(xiàn)出不同程度的組織形態(tài)改變,包括膠原排列紊亂,細(xì)胞密度降低,黏多糖蛋白增多,出現(xiàn)"漲潮"現(xiàn)象等;(2)針刺后24 h與針刺后120 h組纖維軟骨帶厚度比空白對(duì)照組顯著增厚(P0.05);(3)針刺后24 h組與針刺后48 h組的Ⅰ型膠原與Ⅲ型膠原比例顯著低于空白對(duì)照組(P0.05),72 h組最高;(4)針刺后48 h,針刺后72 h與針刺后96 h組的血管內(nèi)皮生長(zhǎng)因子表達(dá)量相較于空白對(duì)照組顯著增高(P0.05);(5)肌張力與膠原比例(r=-0.538;P0.01)及軟骨細(xì)胞密度(r=-0.570;P0.01)呈負(fù)相關(guān)關(guān)系,而肌肉硬度則與血管內(nèi)皮生長(zhǎng)因子表達(dá)量呈正相關(guān)關(guān)系(r=0.613;P0.01);(6)結(jié)果說明,急性損傷后不同開始恢復(fù)訓(xùn)練時(shí)間會(huì)對(duì)髕骨髕腱結(jié)合部的愈合造成明顯的差異,傷后休息72 h開始訓(xùn)練是最為合理的選擇;研究證實(shí)了肌張力及肌肉硬度會(huì)對(duì)細(xì)胞密度,膠原合成及生長(zhǎng)因子表達(dá)存在相關(guān)性,此指標(biāo)測(cè)量方便且無創(chuàng),可應(yīng)用在傷后訓(xùn)練及預(yù)防損傷復(fù)發(fā)等方面,指導(dǎo)運(yùn)動(dòng)員及普通人科學(xué)運(yùn)動(dòng)。
[Abstract]:Background: patellar tendon injury is very common in sports training and the recurrence rate is high. Timely and reasonable recovery training is beneficial to maintain competitive state, but improper recovery training may aggravate the injury. Objective: to investigate the effect of different days of rest training on the healing quality of tendon joint from the angles of histomorphology, growth factor, collagen expression and muscle tension. Methods 40 New Zealand white rabbits were randomly divided into two groups: the blank control group. With the exception of the blank control group, the other groups were treated with acupuncture injury at the point of patellar tendon insertion, and then the established cyclic load training model was used. After 4 weeks of low-intensity training, the animals were killed to get the material, and histopathological hematoxylin eosin was used. After each training, myoton-3 muscle function analyzer was used to collect muscle tension, muscle hardness data and Metamorph graphic processing software for the thickness of fibrochondrocytes, cell density and vascular endothelial growth factor (VEGF) in each group. The cell density of collagen positive staining was calculated quantitatively. Results and conclusion: (1) compared with the blank control group, the training groups showed different degrees of tissue morphological changes, including the disorder of collagen arrangement, the decrease of cell density, the increase of mucopolysaccharide protein. (2) the thickness of fibrous chondrocytes in the groups of 24 h after acupuncture and 120h after acupuncture was significantly thicker than that in the blank control group (P0.05); (3) the ratio of type 鈪,
