【摘要】：目的探討多脾綜合征的MSCT表現(xiàn)及其診斷價(jià)值。方法回顧性分析10例多脾綜合征患者的臨床和CT表現(xiàn)。結(jié)果 9例合并多器官及心血管系統(tǒng)畸形,1例僅合并房間隔缺損。CT表現(xiàn)主要有:(1)多個(gè)脾結(jié)節(jié)。脾臟數(shù)目4~14個(gè);(2)內(nèi)臟異常。全內(nèi)臟反位1例,部分內(nèi)臟反位3例,短胰6例,環(huán)狀胰腺1例,水平肝2例,小腸旋轉(zhuǎn)不良1例,馬蹄腎1例,雙肺左側(cè)異構(gòu)3例;(3)心血管畸形。下腔靜脈肝段缺如,肝靜脈直接匯入右心房,奇靜脈或半奇靜脈擴(kuò)張與上腔靜脈異常連接8例;雙上腔靜脈2例;脾動(dòng)脈開(kāi)口于腸系膜上動(dòng)脈2例;胃左動(dòng)脈開(kāi)口于腸系膜上動(dòng)脈1例;肝總動(dòng)脈開(kāi)口于腹主動(dòng)脈1例;十二指腸前門靜脈2例;合并先天性心臟病2例。結(jié)論CT不僅可清晰顯示脾臟的數(shù)目及位置,而且可顯示合并其他內(nèi)臟異常及心血管畸形,對(duì)多脾綜合征可做出準(zhǔn)確、全面的診斷。
[Abstract]:Objective to investigate the MSCT findings of polysplenic syndrome and its diagnostic value. Methods the clinical and CT findings of 10 patients with polysplenic syndrome were retrospectively analyzed. Results the CT findings of 9 cases with multiple organ and cardiovascular system malformations were as follows: (1) multiple splenic nodules were found only in 1 case with atrial septal defect (ASD). The number of spleen was 4 ~ 14; (2) visceral abnormality. There were 1 case of total visceral inversion, 3 cases of partial visceral inversion, 6 cases of short pancreas, 1 case of annular pancreas, 2 cases of horizontal liver, 1 case of intestinal malrotation, 1 case of horseshoe kidney, 3 cases of isomerism on left side of both lungs. (3) Cardiovascular malformation. The hepatic segment of inferior vena cava was absent, the hepatic vein flowed directly into the right atrium, the azygos vein or half azygos vein dilated with the superior vena cava in 8 cases, the double superior vena cava in 2 cases, the splenic artery opening in the superior mesenteric artery in 2 cases; The left gastric artery opened to the superior mesenteric artery in 1 case, the common hepatic artery to the abdominal aorta in 1 case, the anterior portal vein to the duodenum in 2 cases, and congenital heart disease to 2 cases. Conclusion CT can not only clearly display the number and location of spleen, but also show other visceral abnormalities and cardiovascular malformations. Ct can make an accurate and comprehensive diagnosis of polysplenic syndrome.
