【摘要】：目的調(diào)強(qiáng)放療(IMRT)治療前行CBCT掃描,分析擺位誤差,為確定IMRT放療計(jì)劃中CTV外放PTV提供依據(jù)。方法應(yīng)用VrianIX-4702加速器對(duì)121例腫瘤患者進(jìn)行調(diào)強(qiáng)放射治療,每位患者首次治療前均行CBCT,以后每周進(jìn)行1次CBCT,記錄不同部位腫瘤在左右(X)、前后(Y)、頭腳(Z)3個(gè)方向上的平移差值數(shù)據(jù),計(jì)算并分析首次擺位誤差。結(jié)果頭頸部腫瘤X、Y、Z三個(gè)方向上的首次擺位誤差分別為:(0.046±0.183)cm、(0.052±0.153)cm、(0.021±0.163)cm;盆腔腫瘤X、Y、Z三個(gè)方向上的首次擺位誤差分別為:(-0.040±0.300)cm、(0.183±0.291)cm、(-0.078±0.376)cm。結(jié)論通過(guò)本組CBCT掃描所得擺位誤差分析,推薦IMRT時(shí)頭頸部腫瘤CTV外放PTV(margin)為3個(gè)方向上均為3 mm;盆腔腫瘤margin為X、Y方向上為6 mm,Z方向上為7 mm。
[Abstract]:Objective to analyze the positioning error of CBCT scan before IMRT treatment and to provide the basis for determining CTV PTV in IMRT radiotherapy plan. Methods 121 patients with tumor were treated by intensity modulated radiotherapy with VrianIX-4702 accelerator. Each patient was given CBCTs before the first treatment and once a week thereafter. The error of the first pendulum is calculated and analyzed. Results the error of the first pendulum in the three directions of head and neck tumors was 0.046 鹵0.183 cm ~ (-1) and 0.021 鹵0.163 cm ~ (-1) in the three directions of X ~ (+) Y ~ + Z, respectively, and 0.183 鹵0.291 cm ~ (-1) cm ~ (- 0.078 鹵0.376cm ~ (-1) in the three directions of X _ (1) Y ~ (+) Z for pelvic tumors. The results showed that the error of the first pendulum was 0.183 鹵0.291 cm ~ (-1) for the tumor of head and neck, and the error was 0.021 鹵0.163 cm ~ (-1) for the tumor of the pelvis respectively. Conclusion according to the error analysis of CBCT scanning, it is recommended that the CTV margin of head and neck tumors is 3 mm in three directions and 7 mm in margin of pelvic tumors in XY direction and 6 mm / Z direction in IMRT.
