【摘要】：目的探討艾滋病(AIDS)病人腹部結核的CT表現(xiàn)特征。方法收集2008年4月至2012年3月,重慶市公共衛(wèi)生醫(yī)療救治中心收治的31例AIDS合并腹部結核病人的CT影像及臨床資料,分析其CT影像特征。結果 31例AIDS合并腹部結核病人中,表現(xiàn)為腹部多部位結核,以淋巴結結核最常見(26例),其次依次為結核性腹膜炎(22例)、腸結核(10例)、肝臟結核(5例)、脾臟結核及腹壁結核(各2例)。淋巴結結核主要分布于腹腔干及腰3椎體以上腹主動脈周圍,96.8%環(huán)形強化,其中7例淋巴結腫大融合呈多房狀及花環(huán)樣強化。結核性腹膜炎病人中,4例出現(xiàn)高密度腹腔積液(CT值20HU)。其余部位結核表現(xiàn)無明顯特異性。結論艾滋病合并腹部結核常表現(xiàn)為腹部多臟器受累,淋巴結結核病灶范圍廣泛且易多部位同時發(fā)生。
[Abstract]:Objective to investigate the CT features of abdominal tuberculosis in patients with AIDS. Methods from April 2008 to March 2012, the CT images and clinical data of 31 patients with AIDS complicated with abdominal tuberculosis in Chongqing Public Health Medical Center were collected and their CT imaging features were analyzed. Results among the 31 patients with AIDS complicated with abdominal tuberculosis, the patients presented with multiple abdominal tuberculosis. The most common cases of lymph node tuberculosis were tuberculous peritonitis (n = 22), intestinal tuberculosis (n = 10), hepatic tuberculosis (n = 5), spleen tuberculosis (n = 2) and abdominal wall tuberculosis (n = 2). The tuberculous lymph nodes were mainly located in the abdominal trunk and the abdominal aorta above the lumbar 3 vertebrae with 96. 8% circular enhancement. In 7 cases, the lymph node enlargement and fusion presented multilocular and rosette like enhancement. In 4 patients with tuberculous peritonitis, high density hydroceliac effusion and CT value of 20 HUU were found. The other tuberculosis showed no obvious specificity. Conclusion AIDS complicated with abdominal tuberculosis is often characterized by multiple abdominal organs involvement, and the focus of lymph node tuberculosis is extensive and easy to occur at multiple sites at the same time.
