兒童胸部DR攝影參數(shù)優(yōu)化的研究

【摘要】:目的:比較四組兒童胸部數(shù)字化成像(DR)攝影的輻射劑量和圖像質(zhì)量,優(yōu)化兒童管電壓可行性。方法:該前瞻性研究被新疆大學(xué)醫(yī)學(xué)倫理審查委員會(huì)批準(zhǔn),由兒童本人或監(jiān)護(hù)人或家長簽訂知情同意書。把0至14歲4500例兒童被隨機(jī)分成低千伏(A)、中等千伏(B)、次高千伏組(C)及固定高千伏組(對(duì)照)進(jìn)行數(shù)字化成像(DR)攝影。納入研究4433例兒童胸部正位圖像;排除67例兒童胸部正位圖像,輻射劑量測(cè)量用劑量面積乘積(DAP)。五位放射科診斷醫(yī)生獨(dú)立盲法在工作站上用可視化分級(jí)分析評(píng)分(VGAS)評(píng)價(jià)圖像質(zhì)量。DAP和VGAS均值使用單因素方差分析進(jìn)行統(tǒng)計(jì)學(xué)處理。結(jié)果:C組的DAP均值與低千伏組和中等千伏值的DAP均值比較有統(tǒng)計(jì)學(xué)差異(P<0.05);C組的DAP均值低于對(duì)照組的DAP均值(P<0.05),C組的輻射劑量比對(duì)照組輻射劑量低了18.75%;B值的DAP均值與對(duì)照組的DAP均值比較沒有統(tǒng)計(jì)學(xué)差異(P=0.976);A組的DAP均值大于B值的DAP均值(P=0.000)。對(duì)照組的VGAS均值與A組、B組和C組的VGAS均值比較統(tǒng)計(jì)學(xué)有顯著差異(P=0.000、P=0.000和P=0.000);B組與A組的VGAS均值比較沒統(tǒng)計(jì)學(xué)沒有差異(P=0.334);C組的圖像質(zhì)量略差于B組和A組的圖像質(zhì)量,他們之間的VGAS均值有統(tǒng)計(jì)學(xué)差異(P=0.008、P=0.049)。結(jié)論:C組的管電壓得到的輻射劑量和圖像質(zhì)量是我們可以接受的結(jié)果,并且我們建議應(yīng)用VGAS評(píng)價(jià)圖像質(zhì)量。
[Abstract]:Aim: to compare the radiation dose and image quality of four groups of children with digital chest imaging (DRR), and to optimize the feasibility of tube voltage. Methods: the prospective study was approved by the Medical Ethics Review Committee of Xinjiang University and informed consent was signed by the child, guardian or parent. 4500 children aged 0 to 14 years were randomly divided into three groups: low KV, medium KV, subhigh KV group and fixed KV group (control group). The chest orthographic images of 4433 children and 67 children were excluded, and the dose area product was used to measure the radiation dose. An independent blind method for five radiologists was used to evaluate the image quality by visual grading analysis (VGASA) on a workstation. The mean value of VGAS was evaluated by univariate ANOVA (ANOVA). Results the mean value of DAP in group C was significantly lower than that in group C (P < 0.05) and the mean value of DAP in group C was lower than that in group C (P < 0.05). The mean value of DAP of radiation dose in group C was 18.75g / L lower than that in group C (P < 0.05). There was no statistical difference in the mean value of DAP in the control group. The DAP mean of group A was greater than that of group B (P < 0. 000). There were significant differences between the VGAS mean of the control group and the VGAS mean of group A and group C. There was no significant difference in the mean value of VGAS between group B and group A. There was no statistical difference in the mean value of VGAS between group B and group A the image quality of group C was slightly worse than that of group B and group A. There was a statistically significant difference in the VGAS mean between them, P0. 008 and 0. 049. Conclusion the radiation dose and image quality obtained from the tube voltage of group C are acceptable to us, and we suggest that VGAS be used to evaluate the image quality.
