【摘要】：目的探討胸部CT平掃診斷貧血程度的價值及準(zhǔn)確性。方法測量175例不同程度貧血及非貧血患者的左心室、右心室、室間隔CT值及室隔差值,與36 h內(nèi)患者外周血Hb水平進(jìn)行分析,得出CT平掃診斷貧血程度的最佳閾值。結(jié)果室隔差值與患者Hb水平相關(guān)性最大(決定系數(shù)為0.844,P=0.000)。通過ROC曲線分析,得出當(dāng)室隔差值5.5 HU時,診斷輕度貧血的敏感性為92.9%,特異性較低;當(dāng)室隔差值取9.5~13.5 HU時,診斷中度貧血的敏感性為95.2%,特異性為68.1%;當(dāng)室隔差值13.5 HU時,診斷重度貧血的敏感性為87.5%,特異性為73%。結(jié)論 CT平掃診斷輕、中、重度貧血的最佳閾值分別為5.5~9.5 HU、9.5~13.5 HU、13.5 HU。
[Abstract]:Objective to evaluate the value and accuracy of chest CT scan in diagnosis of anemia. Methods the CT value of left ventricle, right ventricle, ventricular septum and ventricular septal difference were measured in 175 patients with anemia and non-anemia, and the HB levels in peripheral blood of patients within 36 hours were analyzed. The best threshold value for diagnosis of anemia by CT scan was obtained. Results the difference of ventricular septum had the highest correlation with HB level (determination coefficient was 0.844%). By ROC curve analysis, the sensitivity of diagnosis of mild anemia was 92.9 when the ventricular septal difference was 5.5HU, the specificity was low when the ventricular septal difference was 9.5 Hu, the sensitivity was 95.2 and the specificity was 68.1 when the ventricular septal difference was 9.5 Hu, and when the ventricular septum difference was 13.5 Hu, the sensitivity was 95.2 and the specificity was 68.1 Hu. The sensitivity of diagnosis of severe anemia was 87.5 and the specificity was 73. Conclusion the optimal threshold of CT plain scan for diagnosis of mild, moderate and severe anemia is 5.59.5HU9.5U 13.5 Hu 13.5HU respectively.
