【摘要】：目的:研究超低劑量掃描在CT引導下肺部穿刺活檢中的應用。方法:2009年1月至2014年12月來本院200例行胸部CT引導下穿刺的患者分為常規(guī)劑量組(130 k V,90 m As)、超低劑量組(100 k V,7.5 m As),各組100例,并進行穿刺。根據CT機自動記錄的受檢者劑量長度乘積(dose length product,DLP)計算X線有效輻射劑量(effective dose,ED)。比較兩組在輻射劑量、圖片質量、穿刺成功率的差異。結果:超低劑量組和常規(guī)劑量組DLP分別為(25.79±1.13)m Gy·cm和(256.14±4.48)m Gy·cm(P0.05),ED分別為(0.36±0.02)m SV和(3.59±0.06)m Sv(P0.05)。圖片質量均為A級。超低劑量組和常規(guī)劑量組穿刺成功率分別為88%(88/100)為92%(92/100),差異無統(tǒng)計學意義(x2=0.89,P0.05)。結論:超低劑量CT引導下胸部穿刺,可降低受檢者的X線有效輻射劑量,圖像質量可達到臨床CT引導下胸部穿刺的要求,值得提倡和推廣。
[Abstract]:Objective: to study the application of ultralow dose scanning in CT guided lung biopsy. Methods: from January 2009 to December 2014, 200 patients underwent chest CT guided puncture in our hospital were divided into routine dose group (130 kV) and ultra-low dose group (100 kV) with 7.5 m ASL, 100 cases in each group. The effective radiation dose was calculated according to the dose length product DLPs automatically recorded by CT machine. The difference of radiation dose, image quality and puncture success rate between the two groups was compared. Results: the DLP of ultralow dose group and conventional dose group were 25.79 鹵1.13 mGy and 256.14 鹵4.48 mGy respectively. The Ed of P0.05 was 0.36 鹵0.02mSV and 3.59 鹵0.06mSv / mGy, respectively. The picture quality is A grade. The successful rate of puncture in the ultra-low dose group and the routine dose group was 88 / 100) 92 / 100, respectively. The difference was not statistically significant (P 0.05). Conclusion: the chest puncture guided by ultralow dose CT can reduce the effective radiation dose of X-ray, and the image quality can meet the requirement of chest puncture guided by clinical CT, which is worth advocating and popularizing.
